Nursing diagnosis: Decreased cardiac output related to blood loss from vaginal bleeding after delivery as evidenced by a low blood pressure of 90/60 mmHg.
Interventions: Monitor vital signs continuously every 15 minutes for the first hour and every 30 minutes for the next hour. Elevate the patient's lower extremities and observe for continued bleeding and hypotension. Administer IV fluids as ordered to replace blood loss and volume.
Evaluation: After 8 hours of nursing intervention, the patient was able to maintain normal vital signs and the bleeding was reduced, meeting the goal of care.
