WE, EDNA L. CENIZA , married and MILAGROS CONSULAR married, of legal ages,
Filipinos residing at Poblacion , Inopcan, Leyte after being duly sworn to in accordance
with law, depose and state that:

1. We personally know LEONCIA C. MANAPSAL being good neighbors ;

2. We have personal knowledge that LEONCIA C. MANAPSAL died on May 9,2010 at

Inopacan, Leyte ;

3. Her death was not recorded in the Local Civil Registrar as she has no child / children to
do the processing and her immediate relative were presuming that her care giver or the
one attending her when she was yet alive took charge of it ;

4. We declare, confirm and affirm that LEONCIA C. MANAPSAL died on May 9,2010;

5. We are executing this affidavit to confirm and affirm the foregoing facts and for
whatever legal purpose it may serve.
