NOTE:
PERCIVAL TESTING & TECHNICAL CONSULTANCY SERVICES had done the BASIC Ocular Technical Safety
Inspection on all concerned equipment. However, incidents that may cuase the internal failure due to
material deficiencies, beyond our reach/ access such as; metal fatigue, strain, wear and tear and mis-use,
have no assurance of 100% of security.
1 Need to install beacon light. It notify, warn and attract attention to a specific location.
2 Wrong installation of wire rope clip. Saddle is on the incorrect end of the rope. The saddle of the clip rests on the
live end of the rope, while the U-bolt is to rest on the dead end of the rope. The U-bolt part of the clamp crushes
the strand thereby weakening the rope.
3 Wrong installation of socket wedge. Align live end of rope with the center line of pin. Loads may slip or fall if the
socket wedge is not properly installed.
4 Need to install safety latch spring, to prevent the components from becoming unfastened from the hook and
falling on the worker below.
5 Missing boom angle indicator. Need to install. The boom angle indicator is a quick reference for the operator to
know what angle the boom is at.
6 Need to install hook block capacity marking.
7 Kindly monitor kink found on the cable.
8 Need to install anti-two blocking device to prevent contact of hook block and upper sheave. Two-blocking is
one of the most dangerous incidents involving cranes, as it can snap wire ropes and cause dropped loads and
other
catastrophic accidents.
9 Unusual sound when hoisting up.
10 One of the boom end sheave has dent and too thin flange. A condition that could seriously damage the wire rope.
Replace Sheave
11 The unit is " FIT FOR OPERATION ".
12 The following documents should be available in the equipment:
a) Latest certificate/ sticker of inspection.
b) Operator's Log Book to indicate status of the equipment and Maintenance Records.
c) Equipment Operation Manual.
