AFFIDAVIT

I, EMMANUEL SARMENTA GONZAGA, an American Citizen, of legal

age, married and a resident of 14671 Sherman Way Van Nuys, California, United
States of America having been duly sworn in accordance with law, hereby depose
and state:

1. That I was born on December 17, 1953 in Sta. Cruz, Manila City to the
Spouses Felipe Gonzaga and Virginia Sarmenta;

2. That since I was young, I have always been known and called by the first
name “EMMANUEL” and that it is the same first name that I am using in all my
documents and identification papers;

3. That it appears from the records of the Office of the Civil Registrar of
Manila that I was registered with the name of “VIRGILIO” only;

4. That “EMMANUEL” and “VIRGILIO” are the same person as

evidenced by the Certificate of Naturalization issued by the United States of
America (Annex “A” – Certificate of Naturalization dated September 17, 1982)
and Marriage Certificate (Annex “B” – Marriage Contract issued by the Philippine
Statistics Office registered on April 10, 2006);

5. That based on marriage contract, it appears that I was born to the spouses
Felipe Gonzaga and Virginia Sarmenta with the same date of birth as appearing in
my record of birth certificate;

6. That because of the confusion of my parents while recording my first

name in the birth certificate, it appeared that it was “VIRGILIO” that was recorded
instead of “EMMANUEL”;

7. That I was not able to petition the proper agencies in the Philippines for
my change of name since I became an American Citizen on September 17, 1982;

8. That I am executing this Affidavit to certify or attest to the truth of the

foregoing facts and for whatever legal purposes that this Affidavit may serve;
