I, JOVIE F. BALLATAN of legal age, Filipino, with office address at Purok Liberty Gatas
District Pagadian City. After having been sworn in accordance with law, hereby despose and
say as follow:
1) That I am duly elected Corporate Secretary of AMSLIT Training and Assessment
Center Inc., duly organized and existing and by the virtue of the law of the Republic of
the Philippines, with principal office and place of business at Purok Liberty Gatas
District Pagadian City;
2) That I am the custodian of the corporate books and records of the school including the
minutes and resolutions of the board of directors;
3) That at the regular/special meetings of the board of directors of the AMSLIT Training
and Assessment Center Inc. held on 17th day of September, during which a quorum was
present and acted throughout, the following resolution was unanimously approved:

RESOLVED, that any two (2) of the following officers be hereby authorized to sign and
endorse for and in behalf of the school contracts, agreement, statements, reports, and
other documents pertaining to the day to day business operations and activities of the
School .
