Unsafe Lifting/Rigging:

SRM:

It was observed that rigging and lifting activity was

ongoing in batch plant while the competent rigger
was was not present in the area.

Category: (lifting and rigging).

Recommendations:

Immediately informed the rigger to proceed to the

lifting area. Advised crane operator/ supervisor to
ensure that competent rigger is present at all times
when any lifting activity is ongoing, to avoid any
unwanted incident.

Unsafe Chemical Storage:

SRM:

Observed that deisel was stored in an unidentifiable

container without label which is prohibited as per
Amaala Standards and also could lead to
incident/accident at workplace.

Category: (COSHH).

Recommendations:

Immediately advised to the Basmat ul Riyadh to make

proper cable connections and replace the damaged
cables immediatley to minimize the risk of any
incident/accident at wokplace.

Trencher without 3rd party Inspection sticker:

TMC:

Observed TMC bobcat Trencher working at worksite

wihout 3rd party certification evidence with the
equipment which is prohibited as per Amaala
Standards and also could lead to incident/accident at
workplace.
Category: (Heavy Equipment).

Recommendations:
Avised to the TMC to arrange third party certification
of the equipemnt and provide the evidence of the
third party inspection with the equipment to
compliance with the Amaala HSE Standards and
minimize the risk of any incident/accident at
workplace.

GOOD PRACTICES:

1. New Safety Signages Installed at main Highway.

2. Site Nurse visiting the site regularly.
3. Housekeeping has been done by RPCO.
4. Drinking water station has been provided .
5. Housekeeping done by SRM personnel at Batch plant.
