My Choice Rewards (MCR) Benefits are effective the first of the month following your date of hire. All
union benefits will be based on the contract agreement.

• Short term disability is the first of the month following six months of employment and tuition
reimbursement is after one year of employment.

HFHS is a Federal contractor and since the Federal government considers marijuana an illegal
substance, Henry Ford Health System's drug free workplace policy applies. When we conduct your pre-
employment drug screen and marijuana is found in your system, the drug free workplace policy will apply
and your offer of employment will be rescinded.

HFHS wants to protect all of its employees, patients, visitors, and the community. Please bring your
immunization records with you if you have them available, Tetanus, Diphtheria, Pertussis (Tdap) and
Measles, Mumps Rubella (MMR), and COVID-19 Vaccine Immunizations are conditions of employment
and may be administered during your pre-employment health screen. In addition, all employees are
subject to an annual Influenza Vaccination. Annual Influenza Vaccination is a condition of employment
and may also be administered during your pre-employment health screen. Exemptions are available for
medical conditions or spiritual objection. Please ask your recruiter for exemption forms so that you can

complete ahead of your pre-employment health screen. You will be signed up for MyChart during the
pre-employment health screen, please activate it as soon possible. MyChart is an app that allows you to
view you lab results and schedule follow-up appointments from you home computer or cellular telephone.
Upon acceptance of this offer, you will receive multiple emails requesting specific information, we ask that
you complete all requests in a timely manner. Prior to your start date, additional information will be sent to
you via email regarding your orientation and day two training schedule, if applicable.
