Greetings!

In pursuance to MC 2015-02 dated April 30, 2015, please be notified that an inspection of your
cooperative covering the Calendar Year 2021 will be conducted on June 16, 2022
by the Cooperative Development Specialist II of the Authority with the following details:

The Authority allows its field inspectors to conduct virtual inspection during the period of community
quarantine, where applicable, in accordance with Office Order No. 2021-12 dated August 4, 2021.

In view thereof, please ensure that the Chairman, members of the Board of Directors and the
General Manager/Chief Executive Officers or the duly authorized representative/s are present and
available during the opening meeting and exit conference and their absence would constitute a
waiver of their participation therein. Please designate a Focal person that can provide answers to
queries during the conduct of inspection and furnish our field inspector the necessary documents as
provided under Article 52 of RA 9520 as follows:
1. Copy of RA 9520
2. Articles of Cooperation and By-laws
3. Registry of Members
4. Copy of the regulations of the Authority
5. Share books, where applicable; and CY2021 documents, to wit:
a. Books of the Minutes of the meeting of the General Assembly
b. Board of Directors Minutes of Meeting for the year
c. Committee Minutes of Meeting for the year
d. Audited Financial Statement for year ended
e. Development Plan
f. Surety Bond (Updated)
g. Mandatory reports
h. Books for Accounts

inspection, you are hereby requested to submit the necessary documents and email them to us
within
5 days prior to the date of the inspection mentioned above.
