Dear Hon. Jurry J. Basalo,

I, Milagros G. Villarin, 69 years old, widowed, a Barangay Health Worker
assigned in Purok Azucena’s Barangay Taluya for 35 years. As much as I
wanted to serve more years but my health and capacity is limited, I
humbly signify my retirement effective October 7 2022.

It was truly a self-fulfilling and rewarding journey having served my

fellowmen and contributed to the health and welfare of my fellowmen
in a volunteer basis, I am grateful for all the learnings, having an
exception team, for the opportunity and support of all.

I am fully aware that I have met the following requirements stipulated

into the Provincial ordinance NO. 2021-10-084:

 Have at least thirty-five (35) years of uninterrupted as Barangay

Health Worker
 I have secure clearance from the municipal Health board (MHB)
and submit and inclusion to the annual budget.

As I spend time with my family and self-development, I will continue my

advocacy in promotion and support health programs in a volunteer
basis.
