AFFIDAVIT OF LOSS
I, ETHEL GRACE ANYONG PARAJES, of legal age, single, Filipino and a resident
of Macanhan, Carmen, Cagayan de Oro City, after being sworn according to law, hereby depose
and say that:

1. I am the sister of the late GLENDTON AUYONG PARAJES, who died on October
13, 2013 at the Northern Mindanao Medical Center in Capitol Compound, Cagayan de Oro City;

2. Unfortunately, however, the said Certificate of Death of my brother, Glendton Auyong

Parajes that was in my possession, was lost during the flashflood caused by Typhoon Odette
sometime in December 2021;

3. I exerted diligent efforts to locate the said Certificate of Death but the same can no
longer be recovered;

4. I execute this Affidavit to attest the truth of the foregoing statements and in support of
the issuance of the Certificate of Death of the deceased.

IN WITNESS WHEREOF, I have hereunto set my hand this 12th day of October 2022,

I, RENANTE ORNOPIA MIRANDA, of legal age, single, Filipino and a resident of

146 Sta Cruz Village Carmen, Cagayan de Oro City, after being sworn according to law, hereby
depose and say that:

1. I am a depositor of Katipunan Banking Corp.- Capistrano Branch, Cagayan de Oro

City with Joint Account of my minor brother named ALDREEN ORNOPIA MIRANDA under
Time Deposit Account No. 016400007601;

2. Unfortunately however, while in my possession, the said Certificate of Time Deposit

[CTD] was lost from among my personal files and despite diligent efforts exerted to locate, the
same can no longer be recovered;

3. I execute this Affidavit to attest the truth of the foregoing statements and for other
legal purpose this may serve.
