1.0 CHAIRPERSON’S WELCOME REMARKS PM 29/09/2022
The Chairperson welcomed all members present and thanked them for coming, he
emphasized that, it was mandatory for all foreman’s and she reps to attend sheq
meeting as safety is a great concern for everyone. Foremans and She reps should work
hand in glove so that there is 100% adherence to safety regulations, as we zero in to
non-compliance and zero incidents/accidents.
2.0 ADOPTION OF PREVIOUS MINUTES ALL 29.09.2022
Minutes of previous meeting were read and passed as a true record of what transpired
seconded by L. Sibanda.
3.0 MATTERS ARISING FROM THE PREVIOUS MINUTES.
Near misses are important as they are leading indicators, provide learning points and
help us prevent possible accidents/ incidents.
4.0 SAFETY ISSUES ALL Ongoing
 Near miss
All near misses should be written down and reported. Accidents should be reported
promptly.
 Stress Management
There is a need of offsite counsellors to visit the site once a week to provide counselling
services. A street theatre group should perform for employee on site once a week to
revive employees’ morality from emotional stress unfortunately, there is still no money to
hire a theatre group.
 Statistics
A notice board indicating all Tefoma Statistics and manhours to be reacted at the site
office

5.0 INCIDENT RECALL- Learning points from incidents. INCIDENTS SHEQ Ongoing
Accident and incidents to be reported immediately.

 On the 1st of September, the Detour team was offloading Kerbstones from a
delivery truck at Malvern Road intersection, one of the stones fell and hit
Chamunorwa Rupiya on the right leg, inflicting a laceration. First aid was
rendered on site by W. Mbende

 On September 15, 2022, Devotion Kufa was reversing tipper FLT75 when he
accidentally hit a container resulting in minor damage to the left front fender.
The incident was reported to the workshop manager

 On September 19, 2022, Lovemore Gwenere sustained a minor cut to his left
palm while moving a drum at Boka site entrance checkpoint.

 On September 19, 2022, Lucania Mbiriri sustained a minor laceration on her left
finger after she was hit by a piece of firewood at the kitchen area. First aid was
rendered on site.

 On September 19, 2022, Clement Bangura assaulted Talent Mapaya and


Takudzwa Murevanemwe at ramp 4 during shift. Investigations revealed that he
was under the influence of alcohol.

 On September 21, 2022, Isaac Musakwa was stripping shutters at bridge 10


when he accidentally hit himself with a crow hammer on the left leg. First Aid
was rendered to him on site.

 Mellisah Mutsindikwa (Engineer) was driving Toyota Hilux TFM 16 on


Wednesday around 15:30 passing bridge 1 when unexpectedly, a school kid ran
in front of the vehicle trying to cross the road and collided with the left front side
of the car. The kid escaped with injuries to the left leg and the incident was
reported to the Sheq department and Police. The kid was ferried to Parktown
hospital for medical help.

6 SHE INSPECTION ALL Weekly


SHE Inspections to be done on weekly basis. All Supervisors, Foreman, she reps and
Subcontractors to do inspection every week. Supervisors to carry out a weekly
inspection indicating problem areas in their respective work areas. Most of the
supervisors are very reluctant on doing site inspections. Therefore, it should be noted
that inspection is a mandatory thing that needs to be done.

7 SHE EMERGENCY PLAN ALL Ongoing


 We haven’t purchased gongs therefore; we are using whistles in the event of
any emergency on site. Emergency contact numbers have been put up on all
notice boards and in all offices for rapid response.
 There was an emergency mock drill that was conducted on site during the
month testing employee’s response in the event of any emergence and they
responded very well.

8 Site Establishment A Ongoing


Shambir
 Construction of Kitchen facilities and ablution block to cater for Site a
Camp area is still in progress and to be completed.
 Construction of ablution facilities to be completed asap in order to cater
for the high number of employees within the site.

9 Occupational Hygiene and Medical Awareness ALL Ongoing


 A Covid 19
All employees to adhere to covid 19 protocols sanitize, mask up and social distance. All
visitors to be sanitized and inducted on covid 19. Vaccination programme was rolled out
on site and vaccinations are done on site.
 Monkey Pox
Monkey Pox awareness to be conducted on site
 HIV and AIDS
HIV and AIDS to be conducted on site. Engage National Aids Council to educate
employees on site. Supply of condoms to all sites.
 MEASLES
Measles awareness to be conducted on site.
 Pre-Medical Examinations
Pre-employment medical examinations were conducted on site.
 Water Containers
Water containers for all sites were purchased.

10 Environmental Management.

 14 tonnes of biodegradable waste were ferried to Pomona waste


dumping area. An allocated tipper ferries waste to Pomona
approved waste disposal site every Wednesday in accordance with
Waste Management Program for Tefoma.
 Waste generated on site disposed into marked waste segregation

 Used Oil disposal will be done in accordance to environmental
management.

11 Traffic Accommodation
We managed to close the interchange to pave a way for construction of Mbudzi
interchange. However, we still have an interface with the public. Therefore, the drivers
should be encouraged to drive safely. Only exempted cars are allowed to pass through
the construction zone. A designated walkway for pedestrians will be established so that
they don’t disturb construction vehicles.

12 Site Visit
Site visits from OPC, EMA, MOT, ZBC and the herald were conducted during the month.
EMA inspectors commended on the rehabilitation plan for G5 and G7 borrow pits. They
also urged that the exercise being done on dust suppression is of utmost importance
and must be done in accordance with specified requirements.

13.1 Safety Complains PM 29.09.2022

 PPE
There is shortage of PPE on site. However, we were promised that the company is
working on the ppe issue.
 Transportation of Employees
Employees are complaining with the mode of transport they are using from main site to
Mt Hampden.They are Saying tipper isn’t comfortable for transportation of humans so
they are requesting for a kombi.
 Toilets
No adequate toilets in other sections whilst others have no toilets at all.

13.2 Safety directives from Management PM 29.09.2022


 Management was not happy, that so many people are coming to work
whilst they are sick causing the hospital bill to sky rocket therefore,
employees are urged not to report for work if unwell. The hospital is
going to be a trauma centre treating those who get injured at work.
 Earphones to be prohibited because they are causing distractions
therefore penalties to be introduced if found in possession.
 Sleeping on duty is a punishable offence. Peter Kangara(supervisor}

was caught sleeping whilst on duty and let off with a verbal warning.
 Emphases on vendors {if found on your section under your watch you
will be answerable} Company doesn’t want to incur hospital bills in
event of injury
