SUBJECT : Initial Investigation Report, Vehicular Accident

I. AUTHORITY:

1. Standard Operating Procedures of this Police Unit with reference to

Incident recorded in Patrol Section Incident Logbook, Entry No. 2022-004, dated
April 05, 2022.

II. DETAIL:

2. Upon receipt of the report of the Chief of the Patrol Section, the herein
investigator was dispatched by the duty Desk Officer, SPO1 Ernesto D. Rabino ll
to conduct investigation on the alleged Grave Threats being done at the work
place of the complainant.

3. At the scene, the undersigned investigator observed that there is a sign of land
dispute from the owner with title against the defendant who declared himself the
owner of the land verbally.

4. Other details of the incident was that the defendant threatened the worker and
not take adequate legal measures to establish that he is the landowner.

III. RECOMMENDATION:

March 30, 2022.

II. DETAIL:

2. Upon receipt of the report of the Chief of the Patrol Section, the herein
investigator was dispatched by the duty Desk Officer, SPO1 James Dela Cruz and
PO2 Paul Lorenzo to conduct investigation on the case vehicular accident
resulting minor injury that happened at the Barangay Dugo, Camalaniugan,
Cagayan.

3. At the scene, the undersigned investigator observed that there is a sign on the both
parties that they both fast and unnoticed to each other when passing the crossroad.

4. Other details of the incident was they are both unfamiliar on the place because
they are from different town.

III. RECOMMENDATION:

DATE : April 5, 2022

6. AUTHORITY:

7. Standard Operating Procedures of this Police Unit with reference to

Incident recorded in Patrol Section Incident Logbook, Entry No. 2022-003, dated
April 02, 2022.

8. DETAIL:

9. Upon receipt of the report of the Chief of the Patrol Section, the herein
investigator was dispatched by the duty Desk Officer, SPO1 Ernesto D. Rabino to
conduct investigation on the alleged Physical Injury , whose suspect is three
young men one of them is Paulo H. Perez, 19 years old same resident of the
victim.

10. At the scene, the undersigned investigator observed that there is a sign on the
suspects that they were under the influence of alcohol.

11. Other details of the incident were the place where it happened, the area was
hangout place for out of school youth.
