Problem Solving – Quality of Event Checklist

Problem Statement: Does the Problem Statement sufficiently answer the following questions?
Why do we think we have a problem? What are the symptoms?
 What is/is not being experienced?  When did it start?
 Who has reported it?  How many?
 Where is it being experienced?  Is the problem a one-time event, sporadic, or on-going?
 When did the problem occur?  How much is the problem costing?
What information supports these statements?
 Charts  Pictures  Tests/Measurements  Other
How do we know when the problem is solved?
What is expected when the problem is solved?

Containment: What are we doing to minimize the effect of the problem to our customers?
 Containment activities are in place  Effectiveness of activities are documented

Root Cause(s): Identification of the Escape, Occurrence and Systemic Root Cause
 The main potential causes are listed
 Are the Escape, Occurrence and Systemic root causes identified?
Each root cause explains the existence of all symptoms, e.g., what is/is not the problem?
 If not, the true root cause has not been determined
 If the root causes are not adequately determined, review the problem statement, re-evaluate the scope
What information supports these statements?

Corrective Action(s): Activities needed to eliminate the root cause(s) clearly identified/implemented
