Page of the ABM Club relative to the said activity.

I have considered the benefits that my child will derive from his/her participation in the activity

provided that due care and precaution will be observed to ensure the comfort and safety of my child.

I understand that the teacher, school, and DepEd may not be held responsible for any untoward

incident that may happen beyond their control.
