(Service Provider)
Today, _________________, the following has been entered into about execution of kiddie entertainer.

The extent and duration of the contract service wherein Service Provider is going to provide the
following: Package B

 Hosting of the entire duration of the event/party.

 Game Master, Magic Show, Ventriloquism and balloon twisting
 Sound System and 20pcs Game prizes

Which will be executed on _________________at ____________________________________________

time ___________? That Ms./Mr._________________________________________

Will provide the following:

 Game Prizes for the kids and adult

TOTAL PACKAGE OF: Php. 7,000

Down payment of Php. 1,000(Oct.4, 2022) / Balance: Php. 6,000

TERMS OF AGREEMENT

 50% down payment upon confirmation through: Bank Account or Gcash

 Remaining balance should be paid before the event starts.
 In the event of cancellation, the service provider should be notified at least two weeks prior to the event through
email, text or call. 50% of the down payment will be reimbursed.
 Failure to notify the service provider of the cancellation on the said due time, will forfeit reimbursement.
 Additional Php.1, 500 outside Laguna. That in the event of Acts of God, the client has the right to reschedule the
event that will be subject to the availability of the service provider.
 That the service provider shall always be available for any queries or concerns of the client.
 That the service provider shall render a guarantee, that failure to arrive and render services stated herewith on the
said date and time of the event, will not only reimburse the entire 50% down payment but will be subject to pay for
the 50% of the total package due upon.
