1. FORMATIVE:

1. Attendance:
1. Attendance of minimum of 95% of sessions is required
2. Sick leaves will be subject to approval by MOD program Manager (should not exceed 10%
even with approval)
2. Homework Assignment
1. One or more Homework Assignments will be given per domain.
2. Should be submitted by deadlines defined by the instructor (not later than two weeks after
finishing all modules in the domain).
3. No group assignments will be accepted.
4. Assignments should be individualized work.
5. Homework Assignments will have a weighted Mark from the total overall assessment.
6. A feedback will be provided to participants in two months.

 Evaluation and Assessment

2. SUMMATIVE ASSESSMENT:

1. End of Domain Exam:

1. All exams will be done through the Blackboard dedicated to the Training program.
2. End of domain exam (MCQ’s) will be posted on Blackboard within two weeks after the domain.
2. Requirements:
1. Each student shall complete a minimum of 75% of the requirements to pass the program.
3. Final Project:
1. There will be a final longitudinal project at the end of the year to be fulfilled and submitted for final assessment.
2. A QI project will be required from each participant. No group project is accepted.
3. QI Project Topic should be approved by the mentor.
4. The participant will be supervised for the project by pre-determined faculty adviser.

3. TOPIC PRESENTATION:

Present a topic presentation on one of the quality and patient safety emerging topics.
