AFFIDAVIT

I, JAYZEE JOB B. TABANGANAY, of legal age, single, Filipino citizen

and a resident of Purok 4, Bulanao Centro, Tabuk City, Kalinga, Philippines,
after having been duly sworn to in accordance with law hereby depose and
state that:

1. I am the incumbent Sanguniang Kabataan Chairman of

Barangay Bulanao, Tabuk City, Kalinga;

2. I was duly elected as Barangay SK Chairman of Barangay

Bulanao Centro, Tabuk City, Kalinga during the May 14,
2018 and SK Elections;

3. During my incumbency as a barangay official, I was not

employed in a government office nor did I receive any form
a salary or compensation, except honorarium for holding a
position in an ex-officio capacity;

4. FINALLY, I am executing this affidavit to attest to the

truthfulness of the foregoing declarations and for legal
intents and purposes it may serve.
