address at Unit 3C Gold Tower Fortune Hill Condominium, P. Burgos cor. P. Gomez Streets, Barangay
Addition Hills, City of San Juan, Metro Manila hereinafter called the DONOR

office address at J.P. Rizal Extension, West Rembo, Makati City hereinafter called the DONEE.

WITNESSETH

That for and in consideration of a desire to contribute a share for the cause of the education of
Filipino learners through the Adopt-a-School Program of the Department of Education, the DONOR has
freely and voluntarily given, transferred and conveyed by way of donation to the DONEE, its successors
and assigns free and clear of any and all liens and encumbrances whatsoever all its rights.

Details of Donation Amount

Library Books (SEE ATTACHED) Php15,500.00

with a total amount of Php 15,500.00 (Fifteen Thousand five hundred pesos).
