Excavation without barricades:

RPCO:

There is no protection for deep GRPE pipe excavation edges (almost


5 meters in depth), and the bunds that were previously provided are
being removed without prior information or any control measure in
place

Category: (Excavation).

Recommendations:

Immediate action is required by RPCO to provide barricades along


the edges of the deep and unprotected excavations to prevent any
foreseeable accident on site.

Excavation cave-in:

RPCO:

Soil cave-in noticed from the side of some deep excavations for
GRPE pipe installation at AMAALA Apron four area.

Category: (Excavation).

Recommendations:

As per the soil classification appropriate slope to be ensured for


deep excavations to prevent any foreseeable cave-in.

Unsafe work at height:

TMC:

Work at height without the appropriate safety precautions or work


specific PPE was observed in the TMC laydown area. The personnel
engaged in this activity are at risk of falling from heights, which can
result in severe injuries or fatality.

Category: (Unsafe Act).

Recommendations:

It is required to provide proper access and fall arrest systems for all
work at height. A permit should also be obtained prior to performing
the activity.
