INCIDENT PREVENTION BULLETIN

SUMMARY OF INCIDENT

While a contractor pipe fitter was takine

rest in a rest shelter behind the fabrication
shop, a whirlwind lifted c1.nd blew the rest
sbetter over a nearby perimeter- fence and
came to rest approximately 12
meters away. The employee restine in the
shelter was struck and had to be ta.ken to a
nearby
hospita.l for evaluation and treatment. The
affected
employee suffered head

FINDINGS CORRECTIVE ACTIONS

Rest shelter was not 1. Secure all lempor-ary facilities such as

secured.
Sudden strone whirlwind can cause
even heavy temporary structures to
blow over.
Chanees in weather are not commonly
addressed on daily inspections and
work permits.
Not all employees are aware of the
hazards changes in weather can
present.
Emereency response proced1Ses should
include weather retated
scenarios such as hieh winds.

anchors or straps.
2. Conduct weather related toolbox bilks
with employees to increase awareness.
3. Ensure orientations include the
hazards related to natural calamities.
4. Ensure weather related events are
included in emereency response
documents.
5. Monitor weather conditions and assess
the hazards associated al the start
of each shift on work permits and
credible
daily inspections.
6. If weather conditions chanee, reassess
hazards and implement necessary
control measures.
