SUBJECT: COLUMN REPAIRS
SCOPE OF WORK:
1. Due to breakage of the vibrator during concrete pouring, columns C3, D3, and F3 located on
the 2nd floor resulted in honeycombs. We kindly request your assessment of the columns
concerned if these are repairable with epoxy. Please see the attached photos for reference.

NOTES: Should other photos be necessary for approval, they may be provided as requested.
RESPONSE:
COLUMN C3 & D3 CAN BE REPAIRED USING EPOXY INJECTION.
PLEASE REDO COLUMN F3.
