AFFIDAVIT OF GUARDIANSHIP
I, WILSON USMAN AMINULLA, of legal age, single, Filipino, and residing at
Asinan, Kasanyangan, Zamboanga City, Philippines, after having been duly sworn to
in accordance with law hereby deposes and states that:

1. I am the nephew and legal guardian of my late aunt DIUN ULAW

AMINULLA, a senior citizen, who was born January 18, 1953 at Jolo, Sulu;

2. My late aunt DIUN ULAW AMINULLA was under my care and custody
considering that she is single and has no family of her own;

3. My late aunt DIUN ULAW AMINULLA passed away on December 5, 2022 at

Zamboanga City Medical Center, Zamboanga City;

4. I hereby make these statements under oath and in good faith in order to
establish and declare the fact that I am the legal guardian my late aunt
DIUN ULAW AMINULLA;

5. I hereby execute this Affidavit under oath and in good faith to attest to the
truth of the foregoing facts based on personal knowledge and authentic
records for the purpose of processing the Death Certificate of my late aunt
DIUN ULAW AMINULLA and for whatever legal purposes and intents it may
serve.
