Dear Mr. President,

My client, JAIME PEREZ OMPOC of F. Santiago St ., Poblacionsta. Maria, Bulacan has referred to
my office for appropriate action the matter of the Isuzu Elf Truck with Plate No. XJP 645 registered in the
name of your company and driven by your driver, Franky Palaseros Franco which hit and bumped the front
right side portion of my client's Toyota Previas with Plate Number XNX 139 along McArthur Highway
(northbound) Bunlo, Bocaue, Bulacan.

According to my said client, the said incident was caused by the recklessness, imprudence, and lack
of obedience to traffic rules of your driver when driving your vehicle which had caused the damage on
the right side portion of the vehicle of my said client. Since the collection of the damage of my said client
in the amount of P129,831.00 and the amount of P60,000.00 representing the loss of income of said
vehicle for the duration of its repair has been entrusted to my care, through this letter, we are demanding
payment of the said amounts within Seven(7) days from receipt hereof. Failure on your part to comply
with this demand shall constrain us in filing the appropriate civil action against you for the protection of
my client's interest.
