AFFIDAVIT OF DENIAL

I, PHILIP MANUEL DELA CRUZ, of legal age, single, Filipino citizen and
a resident of 5400 Manuel A., Clark Freeport Zone, Pampanga, after being duly
sworn to in accordance with law do hereby deposes and says:

1. That sometime on 16 March 2022, while I was securing a Certification of

No Pending Case in National Bureau of Investigation, City of San
Fernando, Pampanga, I was informed that a case with File No. 16-134 for
Theft was filed against a certain Philip dela Cruz y Manuel;

2. That I am not the person in the above-mentioned case. To prove the

same, attached is the copy of my Certificate of Live Birth as “Annex A”
and a copy of my Voter’s Identification Card as “Annex B”. These are to
prove that the pictures attached and made part of the records of the
above case is not me.

3. That I am executing this affidavit in order to attest to the truthfulness of

the foregoing and for whatever legal intents and purposes it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 11th day of

at Baliwag, Bulacan. Affiant exhibited to me his Voter’s Identification Card
with No. 5527-0031A-E0594PMD10000 bearing his photograph and signature
appearing therein. And I am convinced that he and the affiant is the same
person.
