1.0 INTRODUCTION
1.1 Location & Layout

1.2 Purpose
This provides the procedure for constructing a Steam Turbine Generator & Building, Central
Control Building includes rebar works, form works, anchor bolt setting, concreting works, stripping of
forms, concrete curing, backfilling/compaction for Masinloc Power Plant Expansion Project.

2.0 SCOPE
2.1 This procedure is applicable for constructing a Steam Turbine Generator & Building, Central
Control Building. In accordance with the specification and approved standard drawing for the
Masinloc Coal Power Plant Expansion Project.

5.0 ROLES and RESPONSIBILITIES

5.1 VPCI Project Manager - shall be the overall responsible for the implementation of this
procedure ensuring that works are completed on schedule and in conformity with
drawings and specifications.

5.2 VPCI Project EHS Manager shall ensure safe implementation of this procedure promoting an
Incident and Injury Free environment.

5.3 VPCI & GTC Global Inc. QA/QC Engineer shall be responsible for the implementation
of construction specification and standards and see to it that this
written method statement of the work shall be carried out.

5.4 GTC Global Inc. Construction Manager – shall have the overall responsibility for the
direction, monitoring, coordination and control of all the construction activities attached
and related to concrete works and the overall Health and Safety procedure.

5.5 GTC Global Inc. HSE Engineer along with the Safety Officer shall ensure
implementation of all safety procedures, safety measures related to the nature of works
being carried out and in accordance with the HSE Plan.

5.6 GTC Global Site Supervisor shall be directly involved in the planning and execution of
the activities. He shall be responsible for the preparation of work plan, safety action
plan and safety.

7.1 SURVEYING

7.1.1 Survey Engineer shall establish the lines and levels.


7.1.2 The surveyor shall check that the preceding activities have been approved
7.1.3 All elevation and coordinates values shall be indicated in the drawings or other
documents, unless otherwise stated.

7.2 PERMITTING

7.2.1 Prior to start of any activity, necessary permits should first be accomplished
according to scope of work that are to be conducted.

7.3 INITIAL PLAN


7.3.1 Prior to start of work, place barricades to set boundaries of work and ensuring safe
working area.
7.3.2 Prepare the necessary traffic management plan reflecting the flow of traffic to and
from respective areas.
7.3.3 The sequence of excavation should be carefully planned for heavy machines and
trucks to move easily and safely.
7.3.4 Disposal area shall be located in compliance with site working area.

7.3.5 Access roads shall be provided on strategic sides of the excavation area (North &
East Side). The access road should have a width of 7m. Unsuitable/excess
materials will be disposed outside the 5km. radius.
7.3.6 First to excavate is MF6 where condensate pit is located. MF2 @ Grid AA-AB/A3-A4
will follow. After condensate pit backfilling and compaction is done, STG foundation
@ Grid AB-AD/A2-A6, excavation will proceed together with elevation +1.5m. As
excavation and backfilling is in progress elevation +0.3 will follow and lastly
elevation +2.5m will take place.

7.4 GENERAL EARTHWORK

7.4.1 Excavation less than 1.5 meters deep


7.4.1.1 Excavation approved by AES for backfill shall be stockpiled at allocation
adjacent to the to the construction site as directed by VPCI.
7.4.1.2 Any unsuitable material excavated such as clay or boulders, etc. shall be
removed and properly disposed.
7.4.1.3 All surplus backfill material and unapproved materials shall be transported
off the construction site to a location as designated by VPCI.
7.4.1.4 Sub-contractor shall notify and remove any obstructions encountered
during the excavation.
7.4.1.5 Remove all or any loose material before commencement of any
construction work. GTC shall assume full responsibility for maintaining the
stability of all excavated faces until final acceptance of the foundation
works.
7.4.1.6 If unsuitable soil is encountered at a specified excavation depth, GTC shall
report the condition to VENTANAS to obtain directions for corrective action
before proceeding with the works.
7.4.1.7 Excavation shall be carefully performed not to disturb the bottom of
adjacent foundations.
7.4.1.8 All excavated bottom foundation, bottom trenches and underground lay
pipe shall be compacted or tamped by a jumping jack, rammer or roller
compactor. Over excavation adjacent to the existing foundation must be
avoided, wherever there is over excavation and backfilling of suitable
engineered fill is made on site, compaction test is required.

7.4.2 Excavation exceeding 1.5m deep

7.4.2.1 Excavation exceeding 1.5m in depth have been known to collapse


7.4.2.2 Protection of personnel in excavation with such depth should be prioritized
by providing appropriate signage.
7.4.2.3 Excavation equal to or greater than 1.5m should be cut back to a safe
slope at least 15 %.
7.4.2.4 Provide burlap if deemed necessary. This will remain it stable under
anticipated conditions of work and weather.

7.4.3 Safe Slopes in Excavations

7.4.3.1 Unless the stability of the face is determined by an engineer or other


qualified professional, the safe slope should not exceed:

7.1.3.1.1 1m vertical: 1m horizontal or the angle of repose, whichever is


flatter for soils above the water table.
7.1.3.1.2 1m vertical: 1.5m horizontal or the angle of repose, whichever
is flatter, for saturated or submerged soils, or for excavations
greater than 3m depth.
7.1.3.1.3 Where the slope of an excavation is benched, the maximum
height between benches should not exceed 1.5m, with the
exception of the bench adjacent to the work area, which should
not exceed 1m overall, the total width of the benched
excavation should not less than required in 1 & 2 above.

7.4.4 Sloping Excavation/Trimming


7.4.4.1 Sloping scaling begins at the top of the slope and proceeds down the
slope. Removes or stabilized material on the slope face that is loose,
hanging or creating dangerous situation.

7.4.5 Backfilling / Compaction


7.4.5.1 Backfilled materials shall be free from roots, debris, rock fragments. Prior
to backfilling all backfilled materials shall be inspected and approved by
AES.
7.4.5.2 Survey referencing and marking.
7.4.5.3 Joint inspection will be requested and carried out prior to commencement
of 1st layer backfilling.
7.4.5.4 Backfilling material will be laid and compacted at 300 mm depth for 1st
layer at the bottom of foundation.
7.4.5.5 FDT will be conducted for the 1st layer to comply with project technical
specification requirement for backfilling.
7.4.5.6 Another layer of backfill materials will be laid out above 1 st layer and to be
compacted at 300mm depth.
7.4.5.7 Succeeding layers will be prepared and compacted at 300 mm thickness
until top level providing open space for each layer for FDT.
7.4.5.8 Each layer must be tested for compaction and shall pass a degree of
95%min.compaction of the Maximum Dry Density of soil. Any layer found
not satisfying the required density, a rework shall be done to satisfy the
required density.

7.4.1 Disposal
7.4.1.1 Excavated materials suitable for reuse in the works shall be used for filling
materials, but should be tested and accepted by AES.
7.4.1.2 Excavated materials unsuitable for reuse and surplus shall be removed to
laydown area.

7.5 QUALITY ASSURANCE AND CONTROL


7.5.1 Excavated materials suitable for reuse shall be used for filing materials, but should
be tested and accepted by AES.
7.5.2 Excavated materials unsuitable for reuse and surplus shall be removed to laydown
area.

8.0 Environment, Health and Safety

8.1 All personnel must wear appropriate PPE at all times. Personnel are empowered to stop work if
safety precautions are not in place.

8.2 The Job Site Supervisor shall ensure that all Safety requirements are put in practice, copy of MS
and JHA (Attached) are available on site for guidance and consultation.

8.3 Contractor Safety Representative shall carry out the safety site inspection to ensure the safety
requirements laid out in the safe work plan are followed during and after the completion of the
works.

8.4 Job Site Supervisor and Safety Representative shall be present at all time while works are in
progress.

8.5 Appropriate PTW to be prepared and implemented, modification without OWNER approval is
prohibited.

8.6 Assessed risk as necessary and communicated to concerned parties prior to start of any work.

8.7 Safety briefing / Tool box talk must be conducted by Supervisors prior to start of work in the
language of workforce.

8.8 Incidents and injuries, damaged to property or near misses should be reported.

8.9 Good housekeeping to be maintained during the execution of activity.

8.10 A clear and good communication must be maintained by all personnel involved on the
activity.

8.11 Personnel must be trained/experienced and familiarized with the scope of works to be
undertaken. The location of marshalling point must be identified, emergency evacuation
procedure must be clear to all workers, exercises must be conducted.

8.12 Contractor Safety Manager / Representative shall review Safety Action Plan.
