The PAR numbers from Philippine Business Bank, RCBC Savings Bank, Union Bank of the Philippines, China Bank, and Metropolitan Bank & Trust Company are listed. As a result of being declared lost, these PARs are cancelled and cannot be used as proof of payment to PhilHealth or healthcare providers. People are advised to report any misuse of these lost PARs to PhilHealth.
