is willing now and in the future, to be assigned in any place where his/her services is needed.
_____ 2. Bio-data with most recent original 2 x 2 colored picture.
_____ 3. Xerox copy/ies of diploma/s of college degree/s earned.
_____ 4. Xerox copy of updated transcript of record, with special order if applicable.
_____ 5. Most recent physical examination report, with chest x-ray.
_____ 6. Xerox copy of notarized probationary teacher’s contract.
_____ 7. Xerox copy of baptismal certificate as a Seventh-day Adventist.
_____ 8. Letters of recommendation from the Church Elder and District Pastor.
_____ 9. Church membership record.
_____ 10. Signed conflict of interest form. (Notarized)
_____ 11. Worker’s Undertaking (for regular, probationary and contractual employment) Notarized.
_____ 12. Tax Identification and SSS numbers.
_____ 13. Master Guide Certificate.
_____ 14. If not a Master Guide, a certification by the Mission/ Conference Superintendent of Education
and/or Head of School that no applicant with Master Guide certificate is available, and statement
by the Applicant that within 3 years from the start of probationary employment he/she does not
earn the Master Guide certificate, he/she shall willingly volunteer to resign from the work.
_____ 15. Xerox copy of Civil Service/Professional Board Examination passed, Report of Rating, or PRC License
as a Teacher.
_____ 16. If not a SDA tertiary institution (AUP, NLAC, NVAC) graduate, must have earned 18 or more Bible
units in a SDA college/university as shown in the Transcript of record, or in certification by the
school registrar where the Bible units were earned, and if not an Education graduate, must have
earned 18 or more education units as shown in the transcript of record, or in certification by the school
registrar where the education units were earned
_____ 17. Must be a BEED graduate for elementary teaching, BSE graduate for high school teaching in major
fields.
_____ 18. If not an Adventist tertiary institution (AUP, NLAC, NVAC) graduate, a certification by the
Conference/Mission Superintendent of Education and/or Head of School that no graduate from
the above stated SDA School is available.
_____ 19. Xerox copy of Marriage Contract, if applicable.
_____ 20. Xerox copy of certification that he/she passed the interview for applicants by the Conference Education
Council, after going through the organization’s employment process.
_____ 21. Xerox copy of the result of psychological test given to the applicant.

_____ 27. Xerox copy of Marriage Contract (for those who got married after probationary employment).

Mission Education Council
