SUBJECT: Request for the report of the incident involving Brandon Fabrigate

Mr. Brandon Fabrigate was contracted to do the welding repairs and installations in the
Production Department. The schedule stated clearly that the repairs and installations
should have been finished two days ago and it was reported to our department that they
are not done. Mr. Fabrigate is being cited as being tardy which is unlike him.

After conducting a query on the matter, it was discovered that Mr. Fabrigate was injured
while welding on Tuesday, November 29, 2022. However, it is clear that this information
has not been formally shared with the Production Department or the Human Resource
Department.

Please prepare and send the reports on the above mentioned matter within the next 24
hours .
