Incident Report
Last May 16 to 31, 2018 Cutoff Pedro (Forklift Operator) reported that you have EXCESSIVE TARDINESS.
Tardiness of at least four (4) times or an accumulated tardiness of more than 60 minutes within a calendar month
shall be considered as EXCESSIVE TARDINESS. On this regards, it was clear that she have violated one of our
company policies specially against competency, efficiency and work quality. Based on our company Handbook
shows that the penalty for the said offense shall be Suspension to dismissal depending on the prejudice caused to
the company for the first offense. The employee’s record shows that this is his 1 st commission of the said offense.
Base on the review with our Company handbook, it shows the penalty for the following offenses made
out of or in connection with the aforementioned acts.
From the foregoing, you are hereby asked to explain within Five (5) working days the reason behind the
commission of alleged incident and why the sanction shall not be accordingly imposed upon you.
