Principles of Tooth Reduction
Successful completion of tooth reduction process requires certain principles to
be followed, which are;

1. Mechanical
2. Biological
3. Esthetical

MECHANICAL PRINCIPLES:

Types of occlusal forces:

Path of insertion force Rotational force Tipping force

The design of the prepared tooth should possess adequate:

 Path of insertion.
 Retention & Resistance form.
 Qualities that prevent deformations of the restoration.
 Outline form.
 Finish line.

Factors affecting path of insertion:

1. Reduction of axial surfaces.


2. Roundation of axial line angles.
3. Proximal grooves.

2. Retention and Resistance form:

Retention: is the quality of preparation that prevents the removal of the restoration
along its path of insertion.

Resistance: is the quality of restoration that prevents the dislodgement of the


restoration by forces directed in oblique or horizontal directions.

Tipping Path: is the path along which the restoration could be displaced under the
displacing occlusal forces.

Factors preventing restoration displacement:

The preparation depends on the following retentive feature:

1. Frictional retention between opposing axial surfaces.


2. Retention gained by axial grooves.
3. Retention gained by pinholes.
4. Ledges and Indentations.
5. Retention gained by post inserted in prepared root
canal.
1. Frictional retention between opposing axial surfaces:
a) Axial wall HEIGHT.

b) TAPER of the preparation.

c) Ratio of preparation DIAMETER to axial walls.


d) Circumferential Irregularities.

e) Occlusal Irregularities:

f) Surface Area:

g) Adaptation.
2. Retention Gained By Axial Grooves:
a) V-shaped groove.
b) U-shaped groove.
c) Box-shaped groove.

Criteria Of Ideal Tooth Preparation


1. Occlusal reduction: Minimum recommended clearance is
1 mm on non centric cusp and 1.5 mm on centric cusps.
2. The occlusal reduction should follow the normal anatomic
contours.
3. Axial reduction should be parallel to the long axis of the
tooth with 6 degree convergence between opposing axial
surfaces.
4. The margin should be located supragingivally with a
chamfer configuration approximately 0.5 mm.
5. Functional (centric) Cusp bevel 1.5 mm of occlusal
clearance 45 degrees to the long axis.

4) Occlusal reduction:
o Sufficient clearance of 1 mm on non-functional cusp and 1.5 on
functional cup
o Maintain occlusal anatomy
o Functional bevel.

(Lack of clearance & over occlusal reduction will be considered failing


mistakes).
5) Proximal contacts: to pass the tip of straight probe freely.
(Lack of proximal contact opening will be considered a failing mistake).

point angles)

1- Severe undercuts all-around the preparation
2- Over-reduction (over taper, over thickness of finish line)

Evaluation points: (RUBRICS) :

Each preparation will be evaluated according to the following features:


1. Proximal contacts: to pass the tip of straight probe freely buccolingually and
cervico-occlusally .
a. Clear proximal contacts opening,
b. lack of proximal contact opening will be considered a failing mistake.
2. Incisal edge reduction: amount & its direction in relation to the tooth long axis,
a. Sufficient clearance
b. lack of reduction (under reduction ) & over Incisal shortening will be considered
failing mistakes .
3. Resistance & retention form
a. the opposing axial walls should be converge 10-15o .
b. preparation of labial surface following 2 reduced planes .
c. Adequate inciso-gingival height .
d. Anti-rotational features. Amount of palatal & lingual clearance, lack of
clearance will be considered as a failing mistake.
e. Preparation of cingulum as a definit wall, lack of identification of the cingulum
as a definit wall will be considered as a failing mistake.
f. ,undercuts & over taper will be considered failing mistakes

4. Finish line thickness & continuity:


a. maximum thickness 1,5 mm (shoulder F.L).
b. absence of finish line & lack ofcontinuity will be considered as a failing
mistake.
5. Finish line position: supragingival .
6. Preparation finishing: Finishing of preparation and smoothening of line & point
angles. (smooth surface and finish line, rounded line and point angles)

N.B.
Fatal mistakes: (-50%   )

1- Severe undercuts all-around the preparation


2- Over-reduction (overtaper, over thickness of finish line)
