AFFIDAVIT OF ABANDONMENT

I, OFELIA CABUALAN SANTOS of legal age, Filipino citizen and

currently residing at Valley View Subd., Gulod Malaya, San Mateo, Rizal,
Philippines. After having sworn in accordance with the law, do hereby depose
and STATE:

1. THAT I am married to Jose Sta. Maria Santos.

2. THAT out of the relationship were born the following children:

a. Gerwin C. Santos – 43 years old
b. Jackielyn C. Santos – 38 years old
c. Jaypie C. Santos – 35 years old
d. Jose C. Santos Jr. – 29 years old

3. THAT Jose C. Santos Jr. is a PWD. A copy of his PWD ID with his
Picture and ID No. 0458-110112-1148 is hereto attached as ANNEX
“A”;

4. THAT my partner Jose Sta. Maria Santos abandoned me and my

children in February 13, 2005 and since that time has not
communicated with us nor extended any financial support;

5. Since the abandonment, I have been solely supporting our children;

6. THAT I am executing this affidavit to attest to the truth of the

foregoing facts, particularly, the abandonment of my spouse/common-
law partner.
