APPLICATION TO LEASE MARKET STALL

Dear Mayor Dela Cruz:

I hereby apply for the lease of Stall/Booth in the Meat Section of Teresa Public Market. I
am MICHAEL T. ANN, 45 years of age, and a citizen of the Philippines, residing at Barangay
Dalig, Teresa, Rizal. Should the above-mentioned stall/booth be leased to me in accordance with
the market rules and regulations, I obligate to hold the same under the following condition:

a) That while I am occupying or leasing this stall/booth, I shall at all times have my picture and
that of my helper/s displayed conspicuously inside the stall;
b) I shall keep the stall at all times in good sanitary condition and comply strictly with all
sanitary and market rules and regulations now existing or which may hereafter be promulgated;
c) I shall pay the corresponding rents for the stall/booth or the fee for the stall/booth in a manner
prescribed by existing ordinance;
d) The business to be conducted in the stall/booth shall belong exclusively to me;
e) In case I engage helpers, I shall register them with the Municipal Market Committee but I
shall nevertheless conduct my business and be present at the stall/booth. I shall promptly notify
the Municipal Market Committee of my absence, giving my reason/s thereof;
f) I shall not sell, transfer, mortgage or assumed by others my privilege to the stall/booth or
otherwise permit another person to conduct business therein; and g) Any violation on my part or
on the part of my helpers of the foregoing conditions shall be sufficient cause for the Municipal
Market Committee to cancel or revoke this contract.

of my knowledge.
