CONSULTANCY AGREEMENT

This Contract is made and executed by and between:

RAMIL AMOR, of legal age, Filipino with office address at 23 Oreta St.,
Malinta. Valenzuela City, hereinafter referred to as the CONSULTANT.

And

_______________________________________ registered and existing


under and by virtue of the laws of the Republic of the Philippines,
with principal office at No. 2 Gen. Tibo St., Marulas, Valenzuela City,
herein represented by Its Owner, ______________________,
hereinafter referred to as the CLIENT.

WITNESSETH:

SERVICES COVERED:

ORGANIZING RECORDS

- ASSSITANCE AND GUIDANCE IN establishing, organizing, employ


system of acquiring records of the employees such as 201 files, memos,
legal documents of the company for purposes of complying with the legal
mandates of the law;

- Guidance on facilitation employees benefits such as SSS, Philhealth and


other insurances;

- Preparation and application of systems of payrolls for staff;

- ASSSITANCE AND GUIDANCE in clerical works necessary and required


by the company such as letter/communications to be filed for inter-office
transactions, reports of incidents and legal communications.

- Preparation of Contracts for employment, legal documents for settlements


of benefits and reports necessary for legal offices in connection with
employees welfare.

- Other related documents for the operation of the personnel departments


and others required and requested by the other departments/companies.
CONTROLLING EMPLOYEES CONDUCT

- Advices on Process of Hiring qualified people to positions and job needed


by the company.

- ASSSITANCE AND GUIDANCE To Create good Relationship between


Management and Employees.

- ASSSITANCE AND GUIDANCE in the Implementation of existing rules.

- ASSSITANCE AND GUIDANCE in Implementing discipline to employees


by issuing necessary memo, reminders and restrictions to them

- Promulgating new rules that would suit the existing condition of the
company.

- ASSSITANCE AND GUIDANCE in Communicating with the workers in


times of crisis and make necessary arrangements and settlements in case
of complaints and outburst.

- To introduce strategies that would help the company in dealing with usual
problems of the company with workers.

- To recommend adjustments on workforce in accordance with law.

TROUBLESHOOTING

- ASSSITANCE AND GUIDANCE for the Immediate action during


impending strikes or illegal groupings and sabotage of operation.

- Protect the company from labor cases and employees’ outburst during
critical period by acting as mediator during disputes between employees,
contractor or between employees and management.

- ASSSITANCE AND GUIDANCE In cases of complaint with the


Department of Labor, the Department.

- Recommend possible ways in avoiding damages due to labor problems


including inspections being conducted regularly by Government Agencies.

- Settling problems with SSS, NLRC, DOLE Regional Offices etc.,

- Helping workers to facilitate their benefits and obligations with


Government Agencies in case of death and/or accidents.
DURATION OF CONTRACT:

- 6 MONTH CONTRACT AUTOMATICE RENEWAL


- IN CASE OF TERMINATION, 1 MONTH PRIOR NOTICE IS REQUIRED
- ONCE A MONTH VISIT TO MONITOR THE EXECUTIONS OF THE
STRATEGIES
- Close monitoring through Phone and Messenger Calls
- Can be called via Phone anytime of the working days

RATE: P5,000.00 PER MONTH FOR 6 MONTHS

FIRST PAYMENT SHALL BE 3 MONTHS ADVANCE.

*rate excludes transportation and meal allowances during visits or when


physical presence at the office or outside the office is required.

That the parties hereto shall faithfully comply with the terms and
conditions of this agreement.
