The TARUSAN NATIONAL HIGH SCHOOL, with School Identification Number: 309126 a public High
School, with School Head Address at NARRA-NARRA, TARUSAN, BATARAZA, PALAWAN,
represented in this Agreement by its (Adviser) CHERRY ANN M. PINEDA, FILIPINO, of legal age,
hereinafter referred to as TARUSAN NATIONAL HIGH SCHOOL.

The GRADE 7 ALEXANDRITE PARENTS, duly constituted and registered in the Philippines, with
donors address NARRA-NARRA, TARSUAN, BATARAZA, PALAWAN, represented in this Agreement
by its GRADE 7 PARENTS, EMILY L. JALIL , FILIPINO, hereinafter referred to as TARUSAN

We the undersigned, the teachers of Tarusan National High School, Stakeholders and parents have
come up with a memorandum of agreement/understanding for the school year 2021-2022. This
MOA/MOU is concerning the welfare of the learners as well as the school improvement. Wherein the
teachers, stakeholders, parents must cooperate and support the school programs and projects together
with financial matters, as agrees upon by both parties in a form of meeting.

This agreement may be revised, amended or modified only through a written instrument duly executed
and signed by all parties.
