SAFETY AND HEALTH PROGRAM

1. Executive Summary

APEX Oretrans Corporation, as a contractor for hauling and earth-moving


projects in the mining sector, places a high value on the safety of its employees
and is committed to providing a safe and healthy workplace for all employees,
has developed this program for accident/injury prevention, promoting a drug-
free workplace, avoid illnesses and involve management, supervisors, and
employees in identifying hazards and right application of its controls during the
work process. The annual cost, approved by the management, of implementing
this program for CY 2022 amounted to P1.38 million.

We intend to initiate and maintain complete accident prevention and safety


training. Everybody is held responsible for their safety and others particularly
within the workplace. We shall work together to achieve a culture of Safety and
Zero Injury within the mining operation in consideration with the environment.

2. Company Background

Apex Oretrans Corporation was recently established as a joint venture


company and incorporated and registered with the Securities and Exchange
Commission on January 01, 2022. The primary purpose of this corporation is
to engage in the business of hauling and trucking and other related activities,
through the use of trucks and other vehicles and machines of aggregates,
rocks, stones and other materials in mining areas.

3. Introduction
3.1 Legal Description of the Project

As one of the major contractors for hauling and earth-moving services


for mining sector, APEX Oretrans Corporation ensures its
compliance obligation to its clients and all Government sectors and
agencies. The following agreements/permits covered by the

3.2 Type and Description of the Project

APEX Oretrans Corporation provides equipment and manpower for


moving, discharging, unloading of different aggregates, rocks, stones
and other materials in mining areas. Services provided for mining sectors
are as follows:

Nickel Ore Hauling


Provides equipment and manpower for earth moving services rendered
to Rio Tuba Nickel Mining Corporation, a Subsidiary Company of Nickel
Asia Corporation.

Limestone Hauling
Providing equipment manpower for the discharging of limestone from a
Landing craft tank to the Graymont Philippines Incorporated plant site.

Coal Loading
Providing equipment and manpower for the unloading of coal from a
mother vessel to a landing craft tank.

Pet coke Hauling


Providing equipment manpower for discharging of pet coke from a
Landing craft tank to Graymont Philippines Incorporated plant site.

3.3 Organization Structure

APEX Oretrans Corp. organization consists of four (4) departments


namely: Administrative, Finance, Operations, Mechanical and Safety,
Health and Environment Department. Each department is headed by a
Section Head reporting directly to the Branch Manager. Organizational

4. Leadership and Administration

4.1 Leadership and Administration


It is the basic safety and health policy of this company that no task is so
important that an employee must violate a safety and health rule or take
a risk of injury or illnesses in order to get the job done.

Employees are required to comply with all company safety and health
rules and are encouraged to actively participate in identifying ways to
make our company a safer place to work. Require programs and
examination to our employee to avoid spreading of illnesses like HIV-
Aids, Hepatitis and tuberculosis and the corresponding controls to make
our workplaces safe and healthy to work with.

Supervisors are responsible for the safety and health of their employees
and as a part of their daily duties must check the workplace for unsafe
and unhealthy conditions, watch employees for unsafe and unhealthy
actions and take prompt action to eliminate any hazards.

Management will do its part by devoting the resources necessary to form


a safety committee composed of management and elected employees.
We will develop a system for identifying and correcting hazards. We will
plan for foreseeable emergencies. We will provide initial and ongoing
training for employees and supervisors. And, we will establish a
disciplinary policy to ensure that company safety and health policies are
followed.

The provision of general first-aid and assessment of
health risks to employees when required is done by
the Company’s Occupational Health Nurse. In the
absence of the OHN, Safety Officer and Assistant
Safety Officers are authorized to provide first-aid

4.1.3 Central Safety and Health Committee
APEX Oretrans Corporation management have formed a safety
committee to help employees and management work together to
identify safety problems, develop solutions, review incident
reports and evaluate the effectiveness of our safety program. The
committee is made up of management-designated
representatives and one employee-elected representative each
from the different department of our company.
✓ In addition to the committee responsibilities explained
above, duties of safety committee members include:

o A monthly self-inspection of the area they represent


o Communicating with the employees they represent
on safety issues
o Encouraging safe work practices among co-
workers.

✓ A committee member will be designated each month to


keep minutes on the attached minutes form. A copy will be
posted on the employee bulletin board after each meeting.
After being posted for one month, the minutes will be filed
for one year. The minutes form contains the basic monthly
meeting agenda.

4.2 Organizational Rules


Employees are expected to use good judgment when doing their work
and to follow established safety rules. We have established a
disciplinary policy to provide appropriate consequences for failure to
follow safety rules. This policy is designed not so much to punish as to
bring unacceptable behavior to the employee's attention in a way that
the employee will be motivated to make corrections. The following
consequences apply to the violation of the same rule or the same
unacceptable behavior.
Note: All violations and/or infractions against the provisions of the
Company Code of Conduct are subject to Disciplinary Committee
Hearing/Conference as due process and any employee/s may subject
to immediate termination and/or monetary sanctioning when
administrative and/or safety violation/s and/or accident/incident/s
involvement places the employee, co-workers & company property at a
high-risk of partial/permanent disability or death and/or property
damage/s regardless of the employment status.

4.3 Management and Employee Training

APEX ORETRANS CORPORATION believes that Training plays a vital


role in developing skills of every employee. It is one way of inculcating
employees to be more competent, productive and safe in doing their
assigned task. It improves knowledge, develop employee morale and
enthusiasm, and help ensure greater workforce productivity.

Managers and Supervisors shall be trained and equipped regarding


Safety Rules and Regulations of APEX upon Pre-employment and
during employment on the matters concerning Environment, Safety and
Health compliances, dos and don’ts and Safe Work Practices.

BOSH Training

Every qualified and/or selected Employees/Worker within management’s


discretion based on evaluation of their work performance shall be entitled to
undergo BOSH (Basic Occupational Safety and Health Training) by a duly
registered and accredited DOLE STO (Service Training Organization) upon
availability of training schedule, for personnel training and development
purposes.

LCM Training

Loss Control Management Training for all Managers and Supervisors who
undergone BOSH Training for DOLE and MGB conformance and extent of
Loss Control Implementation within the workplace by duly appointed Loss
Control Coordinators (Training is Subject for schedule depending on
availability and vacancy) conducted by a duly registered and accredited
DOLE STO (Service Training Organization) for personnel training and
development purposes of the company.

Safety Audit Course

Safety Audit Course for Managers and Supervisors who undergone LCM
Training for DOLE and MGB conformance and Standard Safety Auditing
Practices in accord to MGB DAO 2000-98 and DOLE OSHS, conducted by
a duly registered and accredited DOLE STO (Service Training Organization)
for personnel training and development purposes of the company as well.

Employee Training

Employees Training is an essential part of our plan to provide a safe


workplace at mine site and offices. To Ensure that all employees are trained
before they start a task that requires training, we have a trainer who is
responsible to verify that each employee has received an initial orientation
by his or her supervisor, has received any training needed to do the job
safely and that the employee file documents the training. The coordinator
will make sure that an outline and materials list is available for each training
course we provide:

4.4 Good Housekeeping


4.4.1 Good Housekeeping Plan
4.4.1.1 Workers Training
All workers are subjected to undergo Housekeeping
Orientation upon employment. Additional Training and
Orientation are conducted for employees who will be
handling hazardous materials / waste.
4.4.1.2 Routine Maintenance / Housekeeping Schedule
Maintaining of cleanliness in the compound, work areas,
camp sites are done on a daily basis before the start of any
operation. Garbage bins are emptied every morning by the
assigned safety aid. 5s schedule for every department is
done every Saturday of the week where daily tasks are
less.

4.4.1.3 Assignment of Work Responsibilities


Process owners are responsible for maintenance and
housekeeping of their work areas. Common areas such as
Comfort Room, Building Façade, Alleys and Lounge Areas
are under the responsibility of the Safety Aid.
4.4.1.4 Areas of Concern
The maintenance of restricted areas such as
Hazardous materials and waste storage and MRF will be
under the responsibility of the Safety Aid.
4.4.2 Dust Control and Management
Dust control management plans are designed to meet local and
national pollution requirements and ensure the health and safety
of employees, the community, and the environment from fugitive
dust.
4.4.3 Solid Waste Management
The Company recognizes its responsibility to manage wastes
properly and it also recognizes that its more remote operations
may not have adequate and approved third party waste
management facilities available. The Company’s overall
approach will be to minimize waste generation and volumes sent
to sanitary landfills. AOC will implement waste segregation at
source and prohibit use of single use plastic within the company’s
areas of responsibility. Garbage bins for different types of waste
will also be provided and labeled according to classification.
4.4.4 Hazard Control and Management
Hazardous materials will be kept inside sealed containers locked
and caged inside restricted storage area. Only authorized
personnel can access and withdraw hazardous
materials/chemicals as needed. Containers and storage areas
will be labeled and appropriate signages will be put up. Material
Safety Data Sheet will also be placed in accessible areas outside
the storage.

4.5 Health Control and Services


4.5.1 Medical Surveillance
4.5.1.1 Pre-placement Medical Examination
All newly hired employees must undergo a Medical
Examination to ensure that they are physically and
mentally fit to work.
4.5.1.2 Annual Medical Examination during the Quarter
Annual medical examination will be conducted on the first
quarter of the calendar year to all employees, except those
that have undergone and submitted results of medical
examination on the same year.
4.5.1.3 Return to Work Medical Examination
All employees that have not been able to go to work due
to sickness or illnesses for more than 24 hours must
secure return to work medical examination from the OHN.
4.5.1.4 Separation from Employment Medical Examination
All resigning and retiring employees must secure and
submit medical examination at the OHN as part of the
separation from employment requirements.
4.5.1.5 Special Medical Examination
Employees who wish to leave work during operation due
to sickness must undergo medical examination at the OHN
to be provided with the appropriate medical care.
Employees who have been involved in minor and major
accidents without visible injuries must also undergo
medical examination from the OHN or to the nearest
hospital for a more thorough medical examination.
4.5.2 First Aid and Health Care Facilities
Apex Compound and camp sites are provided with first aid box.
There is one treatment area and breastfeeding area provided for
employees that needs treatment and for breastfeeding moms while
on duty. Each safety assistant deployed is provided with first aid kit.

4.5.3 Prevention and Control of Health Domains


4.5.3.1 Human Immunodeficiency Virus and Acquired
Immune Deficiency Syndrome
In conformity with Republic Act No. 8504 otherwise known
as the Philippine AIDS Prevention and Control Act of 1998
which recognizes workplace-based programs as a potent
tool in addressing HIV/AIDS as an international pandemic
problem, this company policy is hereby issued for the
information and guidance of the employees in the
diagnosis, treatment and prevention of HIV/AIDS in the
workplace. This policy is also aimed at addressing the
stigma attached to HIV/AIDS and ensures that the workers’
right against discrimination and confidentiality is
maintained.
4.5.3.2 Tuberculosis
APEX ORETRANS CORP. recognizes that while 80% of
Tuberculosis (TB) cases belong to the economically
productive individuals, it is also treatable and its spread
can be curtailed if proper control measures will be
implemented. As such, AOC has developed TB Policy and
Program for the information and guidance of the
employees.
4.5.3.3 Hepatitis B
APEX ORETRANS CORP. is committed to conform to the
established standards assurance of customer satisfaction,
protection of our environment and health and safety in the
workplaces. The company promotes and ensures a
healthy environment through its various health programs
to safeguard its employees. This program is aimed to
address the stigma attached to hepatitis B and to ensure
that the employees’ right against discrimination and
confidentiality is maintained.

4.5.3.4 Coronavirus Disease of 2019 (COVID-19)


Apex Oretrans Corp. recognizes that COVID-19 pandemic
is a serious public health threat that continues to permeate
all populations and sectors in the country as well as in the
workplace settings. This program aims to provide proper
guidance, appropriate information and action to empower
employees to protect themselves, their colleagues, and
families from the threat of COVID-19.

4.5.4 Promotion of Health Domains


Health promotion and well-being programs focus on the
promotion of health among all workers through safety and health
education or awareness about the different hazard on their
workplace that might or could affect their health during toolbox
meetings prior to exposure or deployment to their assigned duties
and responsibilities.

4.6 Personal Protective Equipment


All Employees on-site must wear appropriate PERSONAL
PROTECTIVE EQUIPMENT as compliance and last-line of individual
protection before and after work to prevent any contact and exposure to
falling, slipping and tripping hazards even on chemical hazardous
substances and electrical hazards as well.

4.6.2 Provision of PPE


All newly hired employees will be provided by the company with the
basic PPE’s such as Hard Hat, Safety Shoes and Reflectorized Vest.
Employees who will be deployed in the hauling and earth-moving
operations will be provided with additional PPE’s depending on the
type of work they will perform. All issuance, storage and request will
be handled by the safety department.

4.6.3 Inventory and Replacement of PPE


APEX Safety department will handle all inventory of PPE’s. A monthly
inventory report will be sent to the Safety Section Head and Branch
Manager for monitoring and approval of requests for replenishment
as needed. Worn-out PPEs before the expected lifespan will be
replaced as long as the old PPE is presented and given back to the
assigned safety personnel. Lost PPE’s due to employee’s negligence
will be replaced but cost will be deducted to their monthly salary.

4.6.4 Feedback Mechanism on PPE Issues and Concerns


PPE Issues and Concerns from employees are discussed during
daily toolbox meeting. Assigned assistant safety officer will raise all
the issues and concerns during the weekly safety department
meeting for action and approval of changes of the safety head.

4.7 Monitoring and Reporting


4.7.1 Planned Inspection
APEX ORETRANS CORPORATION is committed to aggressively
identifying hazardous conditions and practices which are likely to
result in injury or illness to employees. We will take prompt action to
eliminate any hazards we find. In addition to reviewing injury records
and investigating incidents for their causes, management and the
safety committee will regularly check the workplace for hazards as
described as follows:

Annual Site Inspection


Once a year an inspection team made up of members of the safety
committee will do a wall-to-wall walkthrough inspection of the entire
worksite. They will write down any safety hazards or potential
hazards they find. The results of this inspection will be used to
eliminate or control obvious hazards, target specific work areas for
more intensive investigation, assist in revising the checklists used
during regular monthly safety inspections and as part of the annual
review of the effectiveness of our accident prevention program.

Monthly Safety Inspection


Each month, before the regularly scheduled safety committee
meeting, safety committee representatives will inspect all areas for
hazards using the standard safety inspection checklist. They will talk
to co-workers about their safety concerns. Committee members will
report any hazards or concerns to the whole committee for
consideration. The results of the area inspection and any action
taken will be posted in the affected area. Occasionally, committee
representatives may agree to inspect each other's area rather than
their own. This brings a fresh pair of eyes to look for hazards.

Safety Inspection
A necessary activity to be conducted along with the planned
inspections, this activity is for the purpose of ensuring all concerned
department especially the mine operations on their usage and
utilization of deployed Personnel Carriers, Heavy Equipment and
Dump Trucks together with its drivers and operators assuring proper
handling based on Safe Operating Procedures and Guidelines

4.7.1.1 Accident/Incident Investigation


If an employee dies while working or is not expected to
survive, or if any employee is in-patient hospitalized as a result
of a work-related incident, APEX ORETRANS
CORPORATION will contact our client within 24 hours after
becoming aware of the incident. Safety Officer must talk with
a representative of the department. Safety Officer must
report: the employer’s name, location and time of the incident,
number of employees involved the extent of injuries or illness,
a brief description of what happened and the name and phone
number of a contact person.

• DO NOT DISTURB the scene except to aid in rescue or


make the scene safe.

Whenever there is an incident that results in death or serious


injuries that have immediate symptoms, a preliminary
investigation will be conducted by the immediate supervisor
of the injured person/s, a person designated by
management, an employee representative of the safety
committee, and any other persons whose expertise would
help the investigation

The investigation team will take written statements from


witnesses, photograph the incident scene and equipment
involved. The team will also document as soon as possible
after the incident, the condition of any damaged equipment
and any anything else in the work area that may be relevant.
The team will make a written (OHS INCIDENT REPORTING
AND INVESTIGATION FORM) and RCA (Root- Cause
Analysis) of its findings. The report will include a sequence of
events leading up to the incident, conclusions about the
incident and any recommendations to prevent recurrence in
the future. The report will be reviewed by the safety committee
at its next regularly scheduled meeting. When immediate
supervisor becomes aware of an employee injury where the
injury was not serious enough to warrant a team investigation
as described above, the supervisor shall still write an "OHS
Incident Reporting and Investigation Form" and forward them
to Safety Officer

4.7.1.2 Accident/Incident Analysis

occur due to factors concerning and involving basically
PEOPLE, ENVIRONMENT, MATERIALS & EQUIPMENT,
from these factors also we can identify and control Hazards
that may risk the Safety and Health of every worker and the
very environment and communities nearby.

Hence, we come up with an analysis procedure to our work-


related incidents that caused mainly of Near-Miss, Safety
Violations, and Incident (Property Damages) based on our
submitted “Incident Summary and Cases Update”.

This analysis will be focused on eliminating incidence cases


near-misses, violations and damages and continually monitor
each and every activity per shift on the Mine-pit Haul Road,
Access roads, Dumping Areas, Motor pool, other job-related
workplace and mine site. Most observed and reviewed cases
based on the data to be submitted and reported will be
addressed based on the countermeasures identified during
reviews, and depending on the severity of the cases additional
awareness drive and safe practices will be implemented to
eliminate recurrence.

4.7.2 Safety and Health Reporting Requirements

4.7.2.1 Monthly General Accident Report


Monthly General Accident Report shall be completed,
updated and submitted on a monthly schedule. It will be
the main basis for information posted on the safety
statistical board.
4.7.2.2 Monthly Employer’s Report on Accident and Illness
This Report shall be submitted by the employer for every
accident or illness to the Regional Office having jurisdiction
on or before the 20th day of the month following the date
of the accident
4.7.2.3 Minutes of the Meeting of the CSHC
Minutes of the meeting for CSHC will be sent at least on a
quarterly basis by the Safety Head.
4.7.2.4 Quarterly Emergency Drill Report
Emergency Drill Report will be sent at least on a
quarterly basis by the Safety Head.
4.7.2.5 Annual Medical Report
Annual medical report shall be prepared and submitted by
the Company’s OHN on or before March 31 following the
covered year.

4.8 Emergency Response and Preparedness

designated assembly / muster point. Emergency

Earthquake – Drop under a desk or table, cover your head


and hold on. When the shaking stops, team leaders /
foreman / supervisor are to check for damage and
available evacuation routes then begin an evacuation of
their area to the designated assembly location. Evacuation
should proceed as quickly as possible since there may be
aftershocks.
4.8.1.2 Systematic Shutdown of Operation
Fire – All ERT member from mechanical team are tasked
to shut down all running genset or immediately cut-off
power supply from the main source once fire is discovered
inside the compound before evacuating the building.

Earthquake – Drop under a desk or table, cover your head


and hold on. When the shaking stops, All ERT member
from mechanical team are tasked to shut down all

compound before evacuating the building.

4.8.1.3 Control of Hazardous materials


People in the immediate area are to evacuate; close
doors to affected area. Determine the size of the spill and
respond appropriately, according to the spill response plan
Immediately evacuate the area if you are not equipped to
mitigate the spill. Don personal protective equipment,
obtain spill control materials, Contain the spill and dispose
of material correctly. Attend to injured or contaminated
person if safe to do so. Have person with knowledge of
incident or area assist responding emergency personnel.

4.8.1.4 Removal of protection of vital equipment and


supplies
All employees will receive training (P.A.S.S.) on how to
use of fire extinguishers as part of their initial orientation.
A fire evacuation drill will be conducted once or twice a
year during off-season of operation.

4.8.1.5 Designation of Command Control Area


Command Control Area for all types of emergencies is
located in an open space in front of APEX Compound
where it is far from buildings and structures. An evacuation
map for the building is posted at site office. It shows the
location of exits, fire extinguishers, first aid kits, and where
to assemble outside (Evacuation Assembly
Point/Command Control Area).

4.8.1.6 Search and Rescue Plan


Search and rescue consist of three separate operations:
Size up involves assessing the situation and determining a
safe action plan. Search involves locating victims and
documenting their location. Rescue involves the
procedures and methods required to extricate the victims.
These particular operations will be practiced by the trained
members of the ERT at least once or twice a year during
emergency evacuation drills inside the compound.

4.8.1.7 All clear and re-entry procedure


Personnel may re-enter the building only when the Chief
Commanding Officer at the scene of the emergency
situation gives the "All Clear." on all types of emergency
situations. No one is allowed to go home or leave the
premises unless everyone is accounted for.

4.8.3 Emergency Drill

4.8.3.1 Emergency Drill Plan


Emergency drills are conducted quarterly for the present CY.
Specific dates and preparations will be discussed during the
monthly CSHC meeting 4 to 6 weeks prior the agreed
schedule. All participants will be notified on the schedule of
the emergency drill a week ahead if it is an announced
Emergency drill.

4.8.3.2 Emergency Drill Evaluation Assessment


Two (2) weeks ahead of the scheduled Emergency Drill,
Safety Department will conduct an inspection to correct
deficiencies ahead. During the drill. Post-drill meeting will be
headed by the Chief Commanding Officer or invited qualified
external parties to discuss the evaluation. Lastly, a post-drill
form will be completed by the safety head and be sent to all
the participants to be used during CSHC Meeting discussion
of the recently conducted Emergency drill for review and
identification of areas for improvement.

4.8.3.3 Emergency Drill Reporting


Emergency report drill is accomplished by the Safety Head
after the conclusion of the conducted Emergency Drill. The
safety head will submit the duly filled-out Emergency Drill form
along with attachments such as pictures and pre-evaluation
and post-evaluation form duly filled-out by the participants.
The accomplished report will be reviewed by the APEX CSHC
during the next meeting for possible improvements.

4.9 Occupational Safety and Health Management

4.9.1 Safety and Health Meetings


Meeting is a formally arranged gathering with a common purpose.
Meeting with respect to hauling operation is usually made to carry out
the operation smoothly and systematically aiming maximum safety
within workforce and workplaces.

Safety meetings have been practiced regularly for the following major
purposes:

• to present the identified potential hazards of the workplaces;


• to collect ideas and opinion from the group in making
corrective actions both mechanical/engineering and
administrative approaches to the identified potential risks;
• to let everyone, know the safety targets and achievements of
the different departments and institutions;
• to discuss both minor and major accidents encountered;
• to make decisions whether to approve or not approve the
presented suggestions, recommendations, new rules and
procedures, and safety programs and plans.

In APEX Oretrans Corporation, there are four regular safety meetings


that have been practiced in either monthly, weekly, daily or in every
change of shifts. These are the Central Safety and Health Committee
Meeting (CSHC), Departmental Safety Meeting, Section Head
Coordination Meeting and Toolbox Meeting.

Central Safety and Health Committee (CSHC) Meeting


SHC Meeting is a regular monthly meeting participated in by
Manager, section heads, supervisors and employee representatives.

The Branch Manager is the chairman of this committee. Preparations


and organizing the minutes will be made by the safety section. The
meeting is scheduled on a particular Saturday of the month, schedule
of the said meeting will be announced a week before.

The meeting will begin in the morning around 8:30am with safety
inspection of APEX compound or camp sites, with the main objective
of identifying hazard present in the area. The standard flow of the
meeting begins by the words of the chairman at 1:00 in the afternoon.
It follows the presentation and approval of the previous meeting
minutes. Once the minutes have been approved, presentation of
safety department targets and accomplishments will follow. The
following departments will present their concerns in relation to safety
in this order: Operation, Mechanical, HR/Admin, Finance and
employee’s representatives. The coverage of the concerns will be a
day after the previous month meeting up to the day of the meeting.
After the presentation of different departments, discussion of the
safety audit/inspection will follow purposely to review and comments
of the committee on the root cause and the corresponding
countermeasure to eliminate hazard and to prevent recurrence.

Other concerns, such as Emergency Drill schedule and planning will


be discussed as needed. When all agenda have been accomplished,
the Committee chairman will end the meeting.

Safety Department Meeting


Safety department meeting is conducted once a week by the safety
department composed of all the safety officer (including security).
The discussions during this meeting are mostly safety concerns
encountered at the different working areas and its relevant
countermeasures, safety targets, trainings and sharing of experience
with their different field experience.

Section Heads Toolbox Meeting


Section Heads Toolbox Meeting is conducted every day during
weekdays composed of all Department Section Head, Branch
Manager, Head Office Officers and General Manager. Critical
concerns on all department that needs immediate action and
approval of the general manager are discussed during this meeting.

Toolbox Meeting
Toolbox Meeting is a regular meeting conducted by the Operations
Department before the start of any operation headed by Field
Supervisor and attended by the assigned Assistant Safety Officer
and General Associates. This is usually the turnover of assignments
of the outgoing shift to incoming shift. Aside from the safety protocols,
accident, near miss accident, and potential hazards encountered by
the outgoing shift will also be discussed during this meeting so that
everyone would be aware and immediate countermeasures will be
initiated.

4.9.2 Provision for Worker’s Welfare Facilities

4.9.3 Provision for Safety and Health Signages and Information


In all critical areas, safety signs are legibly displayed in most pertinent
locations. Common safety signs are installed in strategic locations
within the compound as well as camp sites within the project owner’s
area of responsibility. Safety signs are usually posted to remind the
workers on how to recognize the different hazards in their working
environment.

4.9.4 Prohibited Acts and Penalties for Violations

The following penalties for violations presented on Table No. 12


applies to all employees who committed safety violations, regardless
of their status.
