PARENT’S CLEARANCE AND WAIVER FORM

This is to certify that I am allowing Mario N. Calingcaling Jr. to go on Class Observation for
120 hours from November 7-25, 2022, and internship for 360 hours from March-May, 2023 at
Cauayan City National High School- Research Annex for the degree in Bachelor of Secondary
Education.

It is understood that I will follow the policies and guidelines set by the school, and abide by
the rules and regulations that may be imposed by the cooperating teacher/supervisor in charge
for his/her welfare and safety.
I fully agree to waive any responsibility on the part of Isabela State University, Cooperating
Schools, and the cooperating teacher/supervisor in-charge in any case of any untoward incident
that may happen to him/her in the duration of class observation and internship.
