I, RYAN CHRISTIAN A. REYES, was born in Cavite City on the 24 th of

December 2000. I am their youngest child among the two children of my parents. I am

currently residing at 501 Camia rd. Sea Breeze subdivision. Caridad, Cavite City

I finished my primary education at Ladislao Diwa Elemantary School, P.Burgos

St. Caridad Cavite City in the school year of 2007-2013. I finished my secondary

education at Cavite National High School at Chief E. Martin St, Caridad, Cavite City in

the school year of 2013-2017. And in June 2017, I finished my tertiary education at San

Sebastian College Recoletos de Cavite at Manila Blvd., J. Felipe Blvd Santa Cruz Cavite

City in the school year of 2017-2019. I took up Bachelor of Science in Computer Science

in Cavite State University – Cavite City Campus at Pulo II, Dalahican, Cavite City.

I would like to acknowledge my sincerest gratitude to the following people for

having their assistance and support in finishing my 240 hours of On-The-Job Training:

Mrs. Teresita R. Tolentino, my On-the-Job training coordinator for being patient

with the trainees and giving them reminders in what they have to do in their internship,

also for securing the trainees’ respective linkage companies;

Mrs. Teresita R. Tolentino, the Department Head of Information Technology, who

gives her utmost support in giving the trainees some advices and tips and for acting as

their second mother in the campus;

Mrs. Maria Christina J. Baesa, the Campus Dean, who gave out her support by

approving and signing the documents every trainee needs to comply with;

Mr. Meynard Villareal, MIS Department Manager, for being hospitable and

making the trainees feel comfortable in their training area, for monitoring their progress

and asks if they need anything;

Mr. Matt Gomez, MIS Department Associate, for giving his support and giving

them advices;

Mr. Jeff Dela Cruz, IS Department Technician, for giving his support, and sharing

his knowledge and giving them advices;

For her unwavering love and support, for meeting all of my material and moral needs, for
her patience and understanding when I'm tired and unable to help with the chores, for
their timeless counsel, and for always being there for me; To my mother;

To my siblings, who are an example to me and who always motivate me to always make
the most of everything, as well as to their proud and steadfast siblings to me; and

Above all, I want to thank Almighty God for giving me a daily life, for my excellent health,
knowledge, and ability to battle and overcome any difficulties in my life.
