CONSENT FOR DEPO-PROVERA
I ____________________________ hereby request Dr. __________________ and/or his/her
associates to administer Depo-Provera. I understand each of the points briefly summarized below,
which has been fully discussed with me and my questions have been answered to my satisfaction.
Please initial after each statement:
1. Depo-Provera should not be taken by women with a known or suspected pregnancy, liver
disease, or any history of blood clots (phlebitis), stroke, vaginal bleeding without a known
cause, cancer of the breast or reproductive organs, or by women who are at risk of
osteoporosis or allergic to the medication in Depo-Provera. _________
2. Studies show that Depo-Provera can cause Osteoporotic changes (bone loss) and that this
may remain for a long period of time after the injections have stopped. The recommendation is
to NOT use Depo-Provera for longer than 2 years. Smoking, alcohol and caffeine increase the
risk of bone loss. _______
3. Depo-Provera is given every 11 to 13 weeks. Waiting beyond 13 weeks for the injection
causes risk of pregnancy. ____________

5. Depo-Provera can take up to 14 days before taking effect, which means that pregnancy could
occur within 14 days of the first injection. A back-up method of birth control is to be used for
14 days after the initial injection. __________
7. Depo-Provera does not protect against any sexually transmitted diseases. ___________
8. Possible side effects include: weight gain, bone loss, decreased sex drive, headache,
dizziness, weakness or fatigue, blood clots. __________

IMPORTANT INFORMATION ABOUT INFLUENZA AND INFLUENZA VACCINE
INFLUENZA VACCINE: Influenza vaccine can protect against certain types of influenza but not against
all virus infections, like the common cold. It is made up of killed influenza viruses and is given by
injection. Because the influenza viruses change, the vaccine also must be changed often. This
year’s vaccine is based on the recommendations by the Center for Communicable Diseases in
Atlanta, Georgia. These strains may cause illness this coming winter. The vaccine should give good
protection to three out of every four persons who take it. In adults, one injection should give
protection, which lasts a year or more. This year, young persons (8 years old and under) need to get
two injections about one month apart to be protected. Any person who received influenza vaccine
last season needs to receive only one dose of vaccine this year, regardless of age.
POSSIBLE SIDE EFFECTS FROM THE VACCINE: Most people have no side effects from the vaccine.
Some have a sore arm for 2 to 4 days. A few have fever, chills, headache, or muscle aches in the
first 48 hours after the injection. As with any vaccine or drug, there is a small chance of serious
reaction and even death. Unlike 1976 swine flu vaccine, recent flu shots have not been linked to
the paralytic illness Guillain Barre Syndrome (GBS). The risk of GBS from the vaccine is very
small. The risk should be balanced against the risk of influenza and its complications. The risk of
death from influenza during a typical epidemic is more than 400 times the risk of dying from any
possible complications of influenza vaccine injections.
WARNING – Some persons should not take influenza vaccine without checking with a doctor.
Those who have fever or significant illness.
Those with allergies to egg (hives, swelling, or breathing difficulty).
Those with a known allergy to thimerosal.
Anyone who has ever been paralyzed with Gullain Barre Syndrome.
PREGNANCY: Doctors are careful about giving drugs or vaccines to pregnant women. Most experts
do not think that influenza vaccine can cause special problems for pregnant women or their unborn
babies, and some believe that pregnant women with chronic medical conditions should be considered
for influenza immunizations. However, a woman who is pregnant or believes she could be pregnant,
should check with a doctor before taking influenza vaccine.

INFORMACIÓN IMPORTANTE SOBRE LA GRIPE Y LA VACUNA ANTIGRIPAL
VACUNA ANTIGRIPAL: La vacuna antigripal puede brindar protección contra algunos tipos de gripe, pero no
