PANDEMIC UNEMPLOYMENT ASSISTANCE MONETARY REDETERMINATION

Dear MARY A COLEMAN,

You filed a claim for Pandemic Unemployment Assistance (PUA) on 6/8/2020. A PUA Monetary Determination
was previously issued in response to your claim filing on 2/1/2021. A redetermination may have been
subsequently issued notifying you of a change in the number of weeks that you are entitled to receive benefits on
the PUA claim.

This reconsideration is being issued more than 30 days after the prior (re)determination, therefore, good cause
for the late redetermination must be established.

State and federal law requires that any time there is a change in your benefit entitlement, a monetary
redetermination must be issued to notify you of the change. This monetary redetermination is being issued on the
Agency’s own motion to notify you of your new benefit entitlement on your PUA claim.

Therefore, good cause for this redetermination in established in accordance with Section 32a(2) of the Michigan
Employment Security (MES) Act.

Due to the high unemployment rate in Michigan, effective November 8, 2020, claimants receiving PUA were
entitled to an additional 7 weeks of benefits. Effective as of February 7, 2021, Michigan is no longer in a high
unemployment period. As a result, you are no longer entitled to the additional weeks of PUA related to the high
unemployment rate.

In accordance with the Continued Assistance Act (CAA) signed into law on December 27, 2020, you continue to
be eligible for the additional 11 weeks of PUA benefits authorized under the CAA.

All previously paid weeks of PUA, regular unemployment benefits, and Extended Benefits (EB), will be deducted
from your total number of weeks of PUA entitlement.

Your claim has been reconsidered.

The prior determination is modified.

Based on information provided, it is determined you do qualify for PUA benefits.

This redetermination is effective beginning March 29, 2020.

Claim Information

balance.
Your weekly benefit amount (WBA) was calculated based on the income amount you provided. If you did not

provide any income, you will receive the minimum PUA weekly benefit amount of 160.00. If you stated in your
application that you had previously had income but did not provide proof of that income, you are required to
submit proof. If the proof is not provided, or is not sufficient to establish your net income, your weekly benefit
amount will be reduced to the minimum of 160.00 per week. UIA will send you an income verification notice.

About PUA
The Coronavirus Aid, Relief, and Economic Security (CARES) Act provided for up to 39 weeks of PUA benefits to
eligible claimants, payable through the week ending December 26, 2020. On December 27, 2020, the Continued
Assistance Act (CAA) was signed into law. The CAA provided for an extension of the PUA program and an
additional 11 weeks of benefits to eligible claimants.

In addition to the CAA, due to the high unemployment rate in the State of Michigan, you may be eligible for an
additional 7 weeks of PUA benefits. As a result of both the high state unemployment rate and the CAA, you may
be eligible for up to a maximum of 57 weeks of PUA benefits. If you have already been paid benefits on a regular
UI claim, extended benefits (EB) or prior weeks of PUA benefits, those weeks paid will be deducted from the
maximum number of weeks that PUA is payable to determine your remaining entitlement.

The additional 11 weeks of PUA available under the CAA are only payable beginning with the week ending
January 2, 2021 through March 13, 2021. If you are not able to collect the full amount of PUA weeks available
before March 13, 2021, and you receive a PUA payment for the week ending March 13, 2021, you may be able to
continue collecting PUA through the week ending April 10, 2021. The period that PUA benefits are payable
between March 14, 2021, and April 10, 2021, is called the phase out period. If you are not entitled to a PUA
payment for the week ending March 13, 2021, or you do not have any remaining weeks available as of that date,
you cannot continue to collect PUA during the phase out period.

Unless the PUA program is extended, benefits under this program will not be paid for any weeks after March 13,
2021, or April 10, 2021 for those who are eligible to receive benefits during the phase out period. You cannot be
paid after April 10, 2021, even if you have remaining benefit entitlement.

Federal Pandemic Unemployment Compensation


The CAA also provides for an extension of Federal Pandemic Unemployment Compensation (FPUC). FPUC
provides an additional $300 per week in supplemental benefits to eligible individuals. The $300 is payable in
addition to your PUA weekly benefit entitlement. FPUC is payable for the weeks ending January 2, 2021 through
March 13, 2021, only, and cannot be paid during the phase out period.

If you are entitled to PUA benefits for any week beginning with the week ending January 2, 2021, you will also be
paid FPUC retroactively for those weeks.

Friend of the Court


PUA and FPUC benefits are subject to Friend of the Court child support deduction orders.

New PUA Claim Requirements


PUA claimants are required to submit proof of employment or self-employment that is the basis of the PUA claim
filing. UIA will send a separate notice requesting the documentation. The request will include additional
information, the deadline date for submission, and examples of acceptable proof. For claims filed or reopened
prior to January 31, 2021, you will have 90 days from the date of the notice to submit your proof of employment.
For new, original PUA claims filed on or after January 31, 2021, you will have 21 days from the date of the
request to submit your documentation.

If you do not submit the proof or if the proof is not sufficient to establish that you were employed or self-employed,
your PUA benefits will stop and you may have to repay PUA benefits that you already received.

In addition to proof of employment, you will be required to verify your identity if you have not already been
required to do so. UIA will send a separate request for your identity verification. The notice will provide additional
information and instructions on how to submit your verification, the deadline for submission, and examples of

acceptable proof. You have 10 days from the date of the notice to submit the requested documents to establish
your identity.

If you do not respond or the documents you provide are not sufficient to verify your identity, your benefits will be
stopped, and you may be required to repay benefits already received.

Bi-Weekly Certification
You must certify your eligibility every two weeks to receive payment. To certify, log into your Michigan Web
Account Manager (MiWAM) at www.michigan.gov/uia. Your first certification should be done online anytime
between June 21, 2020 and June 27, 2020 and then every two weeks thereafter. Your online account is
accessible seven days a week and 24 hours a day. If you do not have a MiWAM, you can create one online at
www.michigan.gov/uia.

You may also certify by phone. Call 1-866-638-3993, Monday through Saturday, 8:00 AM until 7:00 PM, Eastern
Time during your regularly scheduled week.

Appeal Rights
If you disagree with this redetermination you have the right to appeal requesting a hearing before an
administrative law judge. Your appeal must be received within 30 calendar days from the mail date of this
redetermination. If your appeal is filed after the deadline, you must include the reason your appeal is late in your
statement. You can also attach copies of any documents that support your appeal.

You can submit your appeal online at www.michigan.gov/uia through your Michigan Web Account Manager
(MiWAM) and upload documents. If you wish to appeal in writing, complete Form UIA 1733, Appeal of a
Redetermination. This form is located on the website under the Forms link. Include your name, case number and
social security number or Michigan Identification Number (MIN) on documents submitted with your appeal. Fax
the completed Form UIA 1733 and any supporting documents to 1-616-356-0739 or mail to Unemployment
Insurance Agency, P.O. Box 124, Grand Rapids, MI 49501-0124. If you fax or mail your appeal, it must be
signed.

Your appeal must be received no later than April 07, 2021 or this redetermination becomes final.

Under provisions of the Michigan Employment Security (MES) Act, benefits determined payable will be paid even
though another appeal may be filed later. However, if the appeal later determines that you were not entitled to
receive all or part of these benefits, you will be required to repay the benefits improperly received.

IN WRITING: Fax correspondence to 1-517-636-0427 or mail to Unemployment Insurance Agency, P.O. Box
169, Grand Rapids, MI 49501-0169. Include the date, your name, Social Security Number, and signature on all
correspondence. Retain a copy for yourself. Print and keep the confirmation that your Fax was received.

Your information, including but not limited to Social Security Number and Driver's License Number, may be
verified through computer matching programs and shared as authorized by law.

If there is a question regarding the identify of the claim filer, UIA may conduct fact-finding by contacting the filer at
the number provided on the claim. If there is no immediate response, the claimant will have 48 hours to return the
call. If no response is received, a determination regarding identity will be made based on the information.
