    The following PhilHealth Official Receipts (PORs) have been declared lost by Local
    Health Insurance Office (LHIO) Ormoc:

    As a consequence, said receipts are deemed cancelled/withdrawn and cannot be


    presented as proof or records of payments at anytime to PhilHealth and its health
    care providers.

    Please report any incidence of misuse of these PORs to the nearest PhilHealth office
    or call the Treasury Department at (02) 441-7444 local 7459 or 638-8231.
