The lost PAR numbers from China Banking Corporation, Land Bank of the Philippines, One Network Bank, and Philippine Business Bank are listed.     The following PhilHealth Agents Receipts (PARs) have been declared lost by
    the following Accredited Collecting Agents (ACAs):

    One Network Bank 314024892
    Philippine Business Bank 322234314

    As a consequence, said receipts are deemed cancelled / withdrawn and


    cannot be presented as proof or records of payments at anytime to PhilHealth
    and its health care providers.

    Please report any incidence of misuse of these PARs to the nearest PhilHealth
    office or call the Treasury Department at (02)441-7444 loc 7459 or 638-8231.
