Dear Sir,

With reference to the Policy of reimbursement of Telephone/Mobile phone/Internet Charges,

I have taken up the broadband connection to use at home. I have paid Rs. 5998.99 for internet
plan. Internet Plan is valid form 22-05-2021 to 21-05-2022(12 month). Invoice & payment
receipt are enclosed.

Total Amount Total Months Rs Per Month Amount for Period (July-21 to
Dec-21)

Rs. 5998.99 12 Rs. 499.91 499.91* 6 = Rs 2999.46

Total Rs 2999.46

Kindly reimburse amount spent for internet Charges as per the policy notified by HR
department.
