Dear Dr Jones,

RE: Betty Johnson, 81-year-old

Thank you for continuing care of Ms. Johnsons after her total right knee-replacement surgery on
25/02/2015, being discharged today.

Betty returned to the ward with normal vital signs and well-controlled pain. On laboratory, an 80g/L Hb
was found, and she received a transfusion. The coagulation was managed with both warfarin and
Clexane according to protocol. Pain was controlled with oxycodone and paracetamol, and IV iron was
administered.

On the 1/03/2015, Ms Johnsosn started short walks and later was moved to rehabilitation department
on 06.02.2015 where independence was achieved gradually with wound care and hygiene.

Due to the good evolution, the patient is being discharged today with home nursing assistance, an
appointment with physical rehab in 2 weeks therapy in two weeks, and the following medications:
warfarin 4mg, Ferabat 150 mg and oxycodone plus paracetamol.

The as IRN was 3.8 and Hb 112 g/L. It would be greatly appreciated your follow up control and a new
blood work with FBE and IRN control for anticoagulation.
