AFFIDAVIT OF GUARDIANSHIP

I, CHERRY AREVALO ESTRERA, of legal age, Filipino, and resident of Kanagawa-Ken

Yokohama-Shi, Japan, after having duly sworn to in accordance with law, hereby depose and state, that:

1. I am the grandmother of RURI ESTRERA ENCABO who was born on February 9, 2022 in
Japan. A copy of Birth certificate is attached as Annex “A” hereof;

2. Upon the abandonment of mother, SHIELA MARIE ESTRERA ENCABO, I personally take
good care of said baby and provide all his/her/their daily needs including food, cloths, shelter,
etc;

3. I am not imbecile, insane, a vagrant or a vicious person or a habitual drunkard or a habitual

criminal, and have not abandoned, neglected or refused to support said minors or caused them to
commit offenses against the law;

4. By these presents, I am declaring myself as the sole guardian of the aforesaid child and hereby
undertake to faithfully execute the duties of a guardian for the best interest of the ward, and to
provide for the proper care, custody, of the said ward;

5. I am taking full responsibilities as legal guardian of said child pursuant to law and guaranteeing
all acts of the latter with legal effects, including but not limited to performing all and every acts
requisite or necessary or proper to be done for and in their behalf to all intent and purposes as
they might or could lawfully done if legally capacitated to perform acts in accordance with law;

6. That I am therefore executing this affidavit of guardianship to attest to the truth of the foregoing
facts and for whatever legal purpose this may serve.
