Good day!
We, the 4th year BSBA- Financial Management students of Negros Oriental State University, are
conducting a feasibility study about establishing a printing shop at Magallanes Street, Poblacion
9, Tanjay City Negros Oriental with a business name “PrintWorkz Tshirt and Souvenir Printing
Shop.”

We would like to ask for your cooperation in answering this short survey questionnaire. The
purpose of this survey is to understand the perceptions and expectations of customers that will
patronize the products and services.  This survey will only take 3-5 minutes. We assure you that
all responses are recorded confidentially and will be used solely for this study.
