JOINT AFFIDAVIT OF LEGITIMATION

PURSUANT TO RA 9858

We, VANCE B. DE LA PEÑA and LEVIE C. DE LA PEÑA of

both legal ages, Filipinos, and residents of 6158 S Feliciano Street,
Mapulang Lupa, Valenzuela City, after being duly sworn to depose and
state, that:

1. We are the biological parents RICHELLE ANN C. DE LA PEÑA

who was born on September 13, 2009 in Tulbong Midsalip,
Zamboanga Del Sur;

2. We had live together as husband and wife without the benefit of

marriage as contemplated by Article 177 of the Family Code we
begot RICHELLE ANN C. DE LA PEÑA hence her status in the
original birth certificate was entered as illegitimate;

3. We got married on January 16, 2016 which was duly solemnized

by Rev. Joel T. Parungao at #133 Gen. Luna Street, Caloocan
City;

4. We executed this joint affidavit for the purpose of informing

authorities concerned regarding the above facts and that it has
been executed to change the civil status of our child from
illegitimate to legitimate pursuant to Republic Act 9858
otherwise known as AN ACT PROVIDING FOR THE
LEGITIMATION OF CHILDREN BORN TO PARENTS
BELOW MARRIAGE amending for the purpose of Article
177 OF THE NEW CIVIL CODE OF THE PHILIPPINES,
AS AMENDED.
