I hereby give my consent to YES BANK LTD (“Bank”) to use my Aadhaar number and biometric information to establish my identity for availing banking services. I have no objection in the Bank sharing my Aadhaar
details to UIDAI to obtain my demographic information for facilitating Aadhaar based authentication /verification. I have been informed by the Bank about other alternative options available for establishing my identity
including submission of physical KYC documents.
I confirm that I have read all the terms and conditions including schedule of charges related to my account as displayed and updated on the website www.yesbank.in from time to time and confirm that the said terms
and conditions/charges shall continue to remain valid and binding on me in relation to the account held by me and the services/facilities provided by the Bank from time to time. I confirm that the details given herein
are true and correct and acknowledge that the change requested by me/us would be updated in Bank’s records within the committed period from the date of its receipt by the Branch and shall be effective on its
updation ▪ I/we acknowledge that till the change request is effective in the bank’s records, all communications will continue to be sent at the old address/mobile number/e-mail ID. I understand that on updation of the
new mobile number/email id, all transaction and other alerts with respect to my account would be received in this mobile number/email id and I hereby authorize the Bank to contact me for verification call backs or
checks to confirm veracity of any transaction as deemed fit. I shall promptly inform the Bank about any changes in my mobile number/email id from time to time. The bank will not be responsible for return/dishonor of
any old outstanding/unpaid cheques/debits/ requests and which are still in transit and yet to be received/actioned by the bank and not in conformity with the fresh / new signature or new name as and when it is
updated. In case of IEC, Bank shall have the option to reject the request for updation of IEC where its authenticity could not be verified from the DGFT/Ice Gate website.

effective in the systems from that date only. Deliverables if any will be sent to the mailing/communication address as per the latest records available with the Bank. 2. The fresh / new signature(s) would be valid
once the changes are updated in the system. The Bank will not be responsible for return / dishonour of any such old outstanding / unpaid cheque / debits / requests and which are still in transit and yet to be
received/actioned by the Bank and not in conformity with the fresh/ new Signature(s) and / or Operation Instructions Change requests.
