Subject: Letter of request for issuance of Voter’s Certification from the Election
Officer with dry seal.

Dear Sir/Madam,

I am ______________________________, ___ years old, married, and a bonafide

resident of _____________________________. I am writing this letter for the purpose
stated above. My family and I are getting a passport for my wife’s requirement for
employment overseas, in order to do so, I need to present a valid ID during our
scheduled appointment on _______________. Getting a valid ID from other government
agencies takes time and has higher cost. I have a Pag-IBIG ID, but to our dismay, it
wasn’t on the list of acceptable IDs for passport application. One of my wife’s colleague
told her that you no longer issue Voter’s Certification because there is already Philippine
ID issued by the national government but sadly, I still don’t have mine. I am still waiting
for it up to this date and still wonders when it will arrive.

Accordingly, this letter of request is submitted for your kind consideration and approval.
Granting this request would mean so much to me and my family. This is for our
endeavour of attempting to improve our way of life.

Attached herewith is a copy of my Pag-IBIG and company ID, authorization letter, a

copy of my wife’s valid ID and our marriage certificate for your reference.
