AFFIDAVIT

I Mr. Ashutosh Murlidhar Andhale s/o Murlidher And hale Age 28. oath as under years,
occupation business of Civil Contractor do hereby stamp on

That I am residing in A/3, Mahavirdham, Sec-5, shree Nagor, Wagle Estate -locality of District
Thane. Since last 10 -years. That I am the sole proprietor of a proprietor concern name and
style as M/S Ashutosh Ansdhale Civil Contractor Registration No- _______________&
having its office at A/3, Mahavirdham, Sec-5, shree Nagor, Wagle Estate -locality of District
Thane dealing in business of Government, Civil Contracts and ancillary works attached there
of I have passed Diploma/Degree in Civil Engineer in the Year 2018.

Our firm is registered/is to be registered In Class "B-1" under the Govt. Scheme of registration
for unemployed engineers in P.W.D. I further state that at present I am not employee or
partner in any capacity of any firm, and not registered under this scheme any other
department. I am till the date Unemployed Engineer.

The firm is not black listed in any other Govt./Semi Govt. Organization or this Department.

The Affidavit is made for the purpose of registration/renewal of registration as a Civil

Contractor Unemployed Engineer. Whatever stated herein above is true & correct to the best
of my Knowledge and belief.
