General Comments:

MR. KOJI IWASAKI (BTG Division SM)

- Emphasized that Crane Operators should not sit on crane track (unsafe act) and NOT to allow to rest near and around

the crane’s body or around its vicinity.

- Electrical lockout should be complied if working on electricity. Procedure to send out soon.

MR. SAIFUL ULYA (TNJO Deputy PM)

- Upgrade “housekeeping” at LDY-9 and 10 and conduct cleaning at work areas daily.

- Trash bins should be provided where necessary, so as not too scatter garbage out of control.

MR. CHRIS VIAJE (BTG Mech SV)

- Several workers at both LDY 9 and 10 not wearing safety glasses or having NONE at all.

Provision of the required PPE to all workers at the earliest required.
